低温等离子射频切除术治疗声带白斑的疗效分析

摘　　要：目的分析低温等离子射频切除术在治疗声带白斑中的近、远期疗效,并与喉镜下白斑剥脱术进行对比。方法分析2008年1月至2012年1月在我院接受治疗的声带白斑患者的临床资料。依据患者接受的手术方式不同分为观察组（喉镜下低温等离子射频治疗）及对照组（喉镜下白斑剥脱术）。结果本研究共纳入患者101例,其中观察组52例,对照组49例。观察组患者手术时间显著短于对照组（t=4.598,P〈0.001）;观察组患者术中出血量显著少于对照组（t=14.680,P〈0.001）;观察组术后VAS评分显著低于对照组,差异具有统计学意义（t=5.757,P〈0.001）。观察组患者治愈率显著高于对照组,相比较差异具有统计学意义（88.5%,46/52 vs 71.4%,35/49;χ~2=4.609,P=0.032）。观察组术后24h血清IL-1β、IL-6及CRP水平均显著低于对照组（P〈0.05）。观察组患者术后2年累积复发率显著低于对照组,相比较差异具有统计学意义（9.62%,5/52 vs 24.49%,12/49;Logrankχ~2=4.019,P=0.045）。结论在声带白斑的治疗中,低温等离子射频切除术的近、远期疗效可靠,优于传统的白斑剥脱术。Objective Our study was aimed to analyze the clinical efficacy of low-temperature plasma radiofrequency in the treatment of vocal cord leukoplakia. Methods Clinical data of patients with vocal cord leukoplakia received treatment at our hospital from 2008 to 2012 was analyzed retrospectively. Patients were divided into two groups by therapeutic methods, Group A： lowtemperature plasma radiofrequency treatment; group B： stripping. Results A total of 101 patients were analyzed, Group A 52 cases, Group B 49 cases. Group A patients had lower operation time （t=4.598, P〈0.001）, mean bleeding volum （t=14.680, P〈0.001） and postoperative VAS scores （t=5.757, P〈0.001） than these of Group B. Group A patients had higher curative ratio than that of Group B, the difference was statistically significant （88.5%, 46/52 vs 71.4%, 35/49; X2=4.609, P=0.032）. Group A patients had lower levels of IL-1β, IL-6 and CRP when compared to Group B patients （P〈0.05）. Group A patients had lower recurrence rate 2 years ＇after treatment than that of Group B, the difference was statistically significant （9.62%, 5/52 vs 24.49%, 12/49; Log-rank X2=4.019, P= 0.045）.Conclusion The two methods are all effective methods in the treatment of vocal cord leukoplakia, but the low-temperature plasma radiofrequency is better than the ordinary stripping type no matter in the short-term and the long-term curative effect.
