四川省钩端螺旋体病高发县报告病例的准确性调查研究

摘　　要：目的了解四川省钩端螺旋体病高发地区报告钩体病例准确性,为进一步控制钩体病流行提供科学依据。方法采集2012年钩体病高发地区报告临床诊断病例血清进行钩体菌培养,血清学诊断,对钩体病检测结果为阴性的标本进行乙脑、出血热和立克次体检测。结果共采集87例报告病例的血清标本,培养出4株钩体菌,钩体菌培养阳性率为4.6%。采集64份病例急性期和恢复期双份血清进行钩体病检测,其中54例阳性,阳性率为84.4%,采集23份病例单份血清,其中6份钩体菌检测阳性,阳性率为26.1%。对27份钩体检测阴性标本进行乙脑ELISA检测,阳性1例,出血热、立克次体检测结果均为阴性。结论四川省钩体病高发地区临床诊断钩体病准确性较高,但依然需要加强病例双份血清采集和检测。To analyze the diagnosis accuracy of the reported cases in high prevalence counties in Sichuan province and to provide a scientific basis for prevention of leptospirosis,sera of clinical diagnosis cases in three high incident areas in 2012 were collected and examined by microscopic agglutination test（MAT）.Pathogens were isolated from the samples and then cultured,and the strains were identified by MAT.Negative samples were conducted differential detection of Japanese encephalitis,haemorrhagic fever,and rickettsia.Results showed that 87 cases of leptospirosis were reported in three high prevalence counties in2012.All cases sera were examined and cultivated for 4strains of Leptospira.Pathogens culture positive rate was 4.6%.Totally 64 cases of double serum were collected,of which 54 cases were positive for antibodies against Leptospira,positive rate was 84.4%.A total of 23 cases of single serum were testes and 6were positive,with the positive rate of 26.1%.The 27 negative serum samples were conducted Japanese encephalitis ELISA detection,and 1was positive,and the haemorrhagic fever and rickettsia test results were negative.The diagnosis accuracy of the reported cases in high prevalence counties in Sichuan Province is high,but still need to strengthen collecting and testing of double serum for patient.
