改良腹腔镜下完全腹膜外疝修补术实施的可行性及有效性评价

摘　　要：目的探讨改良腹腔镜下完全腹膜外疝修补术的临床应用效果及安全性。方法选取海南省人民医院2012年1月至2014年12月,100例应用改良腹腔镜下完全腹膜外疝修补术治疗的腹股沟疝患者为研究组,选取同期105例传统开放疝环填充式无张力修补术治疗腹股沟疝患者为对照组,观察二组的临床应用效果。结果二组患者的手术时间对比,差异无统计学意义(t=0.452,P=0.217);研究组术中出血量、下床活动时间、疼痛持续时间、住院时间均明显的低于对照组,差异有统计学意义(t=7.418、4.263、3.712、5.225,P=0.005、0.018、0.022、0.011)。研究组手术并发症发生率为8.0%,对照组手术并发症发生率为19.0%,研究组明显低于对照组,差异有统计学意义(χ2=6.518,P<0.05)。结论临床中应用改良腹腔镜下完全腹膜外疝修补术是可行的,与传统的手术比较其具有术后恢复快、创伤小、并发症较少等优点,同时也可减轻患者术后的疼痛,值得在临床中应用。Objective To investigate the clinical efficacy and safety of modified laparoscopic totally extraperitoneal hernia repair. Methods From January 2012 to December 2014,100 patients with inguinal hernia who received modified laparoscopic totally extraperitoneal hernia repair in Hainan Provincial People＇s Hospital were selected as the study group,and other 100 cases of inguinal hernia in same period were performed with open mesh plug hernia repair as control. The clinical effect of both groups were analyzed.Results There was no statistically significant difference in operation time comparing the two groups（ P 0. 05）; blood loss,ambulation time,hospital stay,duration of pain in the study group were significantly lower than those in the control group,and the differences were statistically significant（ P 0. 05）. The incidence of postoperative complications was 8. 0% in the study group,and 19. 0% in the control group. The study group was significantly lower than the control group（ P 0. 05）. Conclusion The clinical application of modified laparoscopic totally extraperitoneal hernia repair is feasible. When compared to traditional surgery,it not only has the advantages of faster recovery,less trauma and fewer complications,etc.,but also can reduce postoperative pain,which is worthy of clinical application.
