摘 要:目的探讨SLE患者血清脂联素与尿蛋白的表达、意义以及相关性。方法将134例SLE患者分为肾脏损害组64例与非肾脏损害组70例,以及47例正常对照组。用双抗体夹心酶联免疫吸附法测定SLE患者及正常对照组血清脂联素;并测定SLE患者的血清白蛋白(ALB)、球蛋白(GLB)、胆固醇(CHOL)、甘油三酯(TG)、尿蛋白,分析各指标间的相关性。结果 (1)血清脂联素在SLE肾损害组(8.22±3.78)mg/L明显高于非肾损害组(3.39±1.72)mg/L以及正常对照组(1.95±0.88)mg/L,差异有统计学意义(P<0.05)。(2)相关分析发现,血清脂联素浓度与尿蛋白、CHOL、TG呈正相关(分别为r=0.691,0.504,0.235,P均<0.01),与ALB、GLB呈负相关(分别为r=-0.569,-0.390,P均<0.01)。结论由于大部分研究提示脂联素是一种保护性蛋白因子,此结果提示,在SLE患者体内的升高可能起到延缓疾病的进展尤其延缓狼疮性肾炎的发生,ALB、GLB、CHOL、TG、尿蛋白的变化可能是反射性引起脂联素增高的原因之一。Objective To investigate the levels of serumal adipenectin and urine protein in Syst-emic Lupus Erythematosus and to explore their roles and correlation in this disease.Methods 134 SLE patients were divided into two groups,one group was SLE nephritis patients(64 cases)and the other group was nonrenal SLE patients(70 cases).There were 47 healthy persons in the control group.ELISA technique were employed to determine the serumal adiponectin concentration in the SLE patients and healthy group;at the same time the serumal albumin(ALB)、globulin(GLB)、cholesterol(CHOL)、triglyceride(TG)and urine protein were detected.Results(1)The levels of serumal adiponectin were increased significantly in the renal SLE patients(8.22±3.78)mg/L compared with nonrenal ones(3.39±1.72)mg/L and healthy group(1.95±0.88)mg/L,(P〈0.05).(2)Plasma adiponectin correlated positively with urine protein、CHOL、TG(respectively r=0.691,P〈0.01;r=0.504,P〈0.01;r=0.235,P〈0.01).It was negatively correlated with ALB、GLB(r=-0.569,P〈0.01;r=-0.390,P〈0.01).Condusion Adiponectin is a protective cytokine,it indicates that the higher level of adiponectin in SLE patients especially who suffer from kidney impairment may delay the disease especially lupus nephritis,The changes in the levels of serumal ALB、GLB、CHOL、TG and urine protein may be a reason why the level of adiponectin increases in the SLE patients.
