摘 要:目的观察了解婴儿乙肝疫苗初免及加强免疫应答与出生体重的关系。方法选择2009年足月出生、出生体重记录清楚,且完成了儿童乙肝疫苗全程免疫和免疫后抗体检测的儿童为观察对象,收集山东省寿光市2009—2014年婴儿乙肝疫苗免疫持久性随访研究资料,对乙肝疫苗初免和加强免疫检测结果进行统计学分析。结果 968名儿童乙肝疫苗全程免疫后抗-HBs保护应答率81.5%,GMT 288.56 m IU/ml;其中,保护应答率随出生体重增加而增高,差异有统计学意义(χ2=16.847,P<0.05);GMT水平与出生体重存在正相关关系(rs=0.083,P<0.05);免疫后5年,抗-HBs<10 m IU/ml的儿童223名,加强1剂次乙肝疫苗免疫后平均GMT 2 303.38 m IU/ml,加强免疫应答与出生体重之间无相关关系(rs=0.011,P>0.05)。结论出生体重是婴儿乙肝疫苗免疫应答的影响因素,出生体重与初次免疫应答存在正相关关系,而与加强免疫则无相关性。Objective To observe and understand relationship between responses of initial and reinforce immunization to hepatitis B vaccine and birth weight in infants. Methods The children full-term born in 2009, with clear records of birth weight and full dose immunization and detection for antibody after immunized by hepatitis vaccine were selected to be obser- vation objects. Research data of follow-up of immune persistence of hepatitis B in infants in Shouguang city, Shandong prov- ince were collected to carry out statistical analysis of detection results of initial and reinforce immunization of the vaccine. Results Rate of protective immune response of anti-HBs after full dose immunization of the vaccine in 968 children was 81.5%, with GMT of 288.56 mIU/ml, and it went up with increase of birth weight. There was statistical significance (X2= 16.847, P〈0.05). Positive correlativity existed between GMT and the birth weight (rs=0.083, P〈0.05). After five-year immuni- zation, there were 223 children with anti-HBs less than 10 mIU/ml, and average GMT after one dose immunization of 2 303.38 mIU/ml. No correlation was found between reinforce immune response and the birth weight (rs=0.011, P〉0.05). Conclusions The birth weight is the influencing factor of immune response to hepatitis B vaccine in the infants. And there is positive correlativity between initial immunization and the birth weight, but no relationship is found between the reinforce immunization and the birth weight.
