甘草泻心汤方证浅析

摘　　要：[目的]探讨甘草泻心汤的方证,为临床选方用药提供依据,以期拓宽甘草泻心汤的临床应用。[方法]通过历代文献的收集,整理《伤寒杂病论》原文与历代医家对甘草泻心汤应用与批注,分析、总结甘草泻心汤的所主之证、君药、煎服方法的特殊性,以及现代临床应用规律。[结果]历代各医家对甘草泻心汤证均有各自独到的见解,甘草泻心汤所主之证为"痞、硬、满、下利、腹鸣、干呕、心烦不得安"等,均为胃虚夹邪之证。甘草泻心汤为治疗寒热错杂之痞症方证之一,方证为中焦虚弱,故重用甘草为君药以建中州。本方煎服法为去滓重煎,属于和解剂的特殊煎法。本方现代可用于白塞氏综合征、复发性口腔溃疡等辨证属甘草泻心汤证者。[结论]甘草泻心汤不仅仅局限于《伤寒杂病论》的条文所主之证,甘草泻心汤及其加减方已广泛应用于现代临床,辨方证论治对临床实践具有重要的指导意义。启示后学者运用甘草泻心汤时当辨证论治,辨方论治,以扩大方剂的应用范围。[Objective] To analyse and summarize the features of Gancao Xiexin Tang in this formulation ＇s prominent indications, monarch ingredient,special decoction method and administration, and figure out its application pattern in modern medical field. [Method] By collecting all the medical datum of Gancao Xiexin Tang in traditional Chinese medicine, we explored and researched the original text of Treatise on Cold Damage and Miscellaneous, and old expertise＇s experiences and endorsement of the formulation. [Result] As to the specific functions of Gancao Xiexin Tang, every physician in the Chinese medical history did convey his own views, but there were some consensus, such as this formulation ＇s appropriate indications, like＂stuffiness,distention, hardness, diarrhea, barborygmus, retching, and vexation ＂, all the symptoms can be classified into syndrome of stomach deficiency mixed with excess. As we know, Gancao Xiexin Tang is one of the empirical formulae to treat stuffiness symptom which belongs to syndrome of cold intermingled with heat. Due to the insufficiency of the stomach, high dose of gancao, as monarch ingredient, was added to tonifying stomach. And its decoction method is decocting again after deslagging which as utilized in harmonizing prescription. In previous study, Gancao Xiexin Tang has been prescribed for a wide variety of diseases, such as Behcet＇s disease, recurrent oral ulceration that accord with syndrome differentiation. [Conclusion]The usage of Gancao Xiexin Tang is more than the major indications that written in the original text of Treatise of Cold Damage and Miscellaneous, under the instruction of treatment based on syndrome differentiation theory, the prescription and its modifications are widely applied in modern clinic. This article emphazies the importance of syndrome and prescription-syndrome differentiation during the application of Gancao Xiexin Tang and enlightens us how to use it flexiby in clinical practice.
