阿托伐他汀与克拉霉素相互作用致严重肌毒性的病历分析

摘　　要：目的通过本次病例分析,揭示临床使用他汀类药物与克拉霉素合用产生严重肌毒性的相互作用。方法临床药师通过综合分析文献,根据不良反应判断标准等分析,得出阿托伐他汀致四肢肌肉无力、酸软的不良反应的结论,协助医师制定治疗方案并提供药学监护。结果此病例为他汀类与克拉霉素相互作用致严重肌毒性不良反应。立即停用阿托伐他汀、克拉霉素及其他药品,换用普伐他汀继续治疗基础疾病,未再发生相似不良反应。结论临床使用他汀类药物需重视其安全性,特别注意与克拉霉素等其他药物的相互作用,减少严重不良反应的发生。OBJECTIVE To provide warning for clinical use of statins with clarithromycin by reporting this case.METHODS By analyzing literatures and referring to adverse reaction judgment standards,clinical pharmacists drew a conclusion that atorvastatin can result in weakness and lassitude of limb muscles,which assists clinicians to formulate therapeutic regimens and provides pharmaceutical care for patients.RESULTS This was a typical drug interaction between atorvastatin and clarithromycin which lead to severe statin associated myopathys. After stop using atorvastatin,clarithromycin,and other drugs,then switch pravastatin instead of atorvastatin for basic diseases treatment,similar adverse drug reaction did not happened. CONCLUSION Statin security and drug interaction especially combined with clarithromycin should be attaching importance in clinical use so as to reduce serious adverse reactions.
