下肢动脉硬化闭塞症患者介入手术前情绪状态及原因的调查研究

摘　　要：目的调查介入手术术前患者的情绪状态，并探讨其内在原因。方法连续收集46例患者，采用心境形容词简表（MACL）和整体的焦虑—镇定的认知程度评分将患者分为镇定组32例和焦虑组14例，然后对镇定和焦虑的原因进行面对面的针对性调查研究。结果镇定组整体评分4～6分，MACL评分（3．30±0．39）分；焦虑组整体评分1～3分，MACL评分也同样显示情绪低落，得分（2．51±0．50）分；两组比较差异有统计学意义（Z=355．0，P〈0．01）。术前镇定感与对医护人员的信任和手术预后的良好预期相关；而焦虑感则主要与对不良预后的恐惧和对治疗选择的不确定性相关。结论在采用有创性介入治疗前，增加患者对自身疾病相关知识、治疗现状和治疗流程的了解，有利于增进患者对治疗选择的认可度，同时也有利于患者减少焦虑和增加正面情绪。Objective To investigate the emotional states before interventional procedure among patients with arterial occlusive disease of lower extremities and explore the intrinsic reasons. Methods We constantly collected 46 patients to be divided into calm group （ n = 32） and anxiety group （ n = 14） by the shortened version of mood adjective che^k list （MACL） and the overall assessment of the perceived degree of calmness-anxiety, then investigated the reasons of calm or anxiety correspondingly face-by-face. Results The overall score of calm group was 4 to 6, in which MACL score was （3.30 ±0.39） ; the overall score of anxiety group was 1 to 3 with MACL score （2.51 ± 0.50） showing depression mood; differences between two groups were statistically significant （Z = 355.0, P 〈 0. 01 ）. The sense of calm before operation had correlation with the trust of medical staffs and postoperative good prognosis; the sense of anxiety was mainly correlated with the fear of poor prognosis and uncertain treatment choose. Conclusions Before the implementation of interventional procedure, increasing patient＇s knowledge, current treatment and treatment process of own disease can promote patient＇s recognition degree of treatment choose, and be benefit for the decreasing of anxiety and increasing of positive emotion.
