黄蒲通窍胶囊治疗血管性痴呆疗效观察

摘　　要：目的 :观察黄蒲通窍胶囊治疗血管性痴呆患者的临床疗效。方法:将60例血管性痴呆患者随机分为治疗组30例(予以黄蒲通窍胶囊,0.35g/粒,4粒/次,3次/d,空腹服用)和对照组30例(予以多乃哌齐5mg/片,5mg,睡前服用)。2组治疗前后均进行ADAS-cog、MMSE评分、ADL评分、SDSVD评分;结果 :(1)临床疗效:2组患者临床总疗效比较无显著性差异,表明中药治疗组与西药对照组疗效相当(P>0.05);(2)认知功能与日常生活能力:与治疗前相比,治疗组和对照组治疗后ADAS-Cog分值均有降低(P<0.01),但2组治疗后相比无统计意义(P>0.05),治疗组和对照组治疗后MMSE分值均有提高(P<0.01),但2组治疗后相比无统计意义(P>0.05),说明黄蒲通窍胶囊与盐酸多奈哌齐在改善血管性痴呆认知功能方面效果相当,治疗组和对照组治疗后ADL指数分值均有降低(P<0.01),但2组治疗后相比无统计意义(P>0.05),说明黄蒲通窍胶囊与盐酸多奈哌齐在改善患者日常生活能力方面疗效相当;(3)中医证候:与治疗前相比,治疗组治疗后SDSVD分值降低(P<0.01),但对照组治疗前后相比差异无统计学意义(P>0.05);2组治疗后相比有统计意义(P<0.05),说明黄蒲通窍胶囊在改善中医证候方面,疗效优于盐酸多奈哌齐。结论:多奈哌齐和黄蒲通窍胶囊均能够延缓血管性痴呆患者的进程、改善症状,具有明显的改善血管性痴呆患者的日常生活能力、提高生活质量的作用;但黄蒲通窍胶囊在改善患者中医证候积分方面效果优于多奈哌齐,且安全无明显副作用。Objective： To observe the curative effect of vascular Huangpu Tongqiao Capsule for patients with vascular dementia.Methods： A total of 60 patients with VaD was divided into two groups by computer random number generator, followed by a 3-month treatment with I-Iuangpu Tongqiao Capsule （n=30）and donepezil （n=30） except for basic treatment respectively.The primary outcome measure was Alzheimer＇ s Disease Assessment Scale cognitive sub-scale （ADAS-cog）. The secondary outcome measures included the Activities of Daily Living（ADL）, the Mini Mental State Examination （MMSE） and the scale for the differentiation of syndromes of vascular dementia （SDSVD）. Results： （1） clinical efficacy： The to- tal effective rate of Donepezil group is 73.l and Huangpu Tongqiao Capsule 70.8, between the two groups, the difference was statistically insignificant（P〉0.05）. （2）cognitive function： The two groups showed a significant deterioration of ADAS- cog scores from baseline after treatment （P〈0.01）, Adverse events were rarely found in two groups. There was a significant increase in MMSE scores for the two groups from baseline after treatment（P〈0.01）, conversely. （3）Quality of Activities of daily life ： there was no significant difference between the two groups in ADL scores（P〉0.05）. （4）Symptoms of Tradi- tional Chinese Medicine： The Huangpu Tongqiao groups showed a significant decline of SDSVD scores from baseline after treatment（P〈0.01）, while there was no significant difference in Donepezil group（P〉0.05）, there was no significant differ- ence between the two groups after treatment. Conclusions： Huangpu Tongqiao Capsule and Donepezil may improve cogni- tion and increase the abilities of Daily Living of vascular dementia patients, and Huangpu Tongqiao Capsule can also ame- liorate patients＇ symptoms evaluated by Traditional Chinese Medicine syndrome, meanwhile it is safe and well tolerated.
