大剂量盐酸氨溴索联合纤维支气管镜吸痰治疗重症肺炎的疗效和安全性分析

摘　　要：目的分析针对重症肺炎患者应用大剂量盐酸氨溴索联合纤维支气管镜吸痰治疗的作用以及效果。方法选择我院ICU收治的重症肺炎患者60例作为研究对象,将患者随机分为研究组和对照组,各为30例进行分组治疗,在常规治疗基础上,研究组实施大剂量盐酸氨溴索联合纤维支气管镜吸痰治疗,对照组实施常规剂量盐酸氨溴索联合振动排痰仪排痰治疗。将两组患者的临床疗效情况进行对比。结果完成治疗后,两组患者的动脉氧分压(Pa O2)、动脉血氧饱和度(Sa O2)和氧合指数(Pa O2/Fi O2)等血气指标数据均显著升高,研究组显著高于对照组(P<0.05);两组患者白细胞(WBC)、C反应蛋白(CRP)以及血降钙素原(PCT)等炎性指标均显著降低,研究组上述指标明显低于对照组(P<0.05)。在接受机械通气时间方面,研究组也显著少于对照组(P<0.05)。结论针对重症肺炎患者应用大剂量盐酸氨溴索联合纤维支气管镜吸痰治疗,可以产生显著的效果,安全性高,缩短机械通气时间,有利于患者预后,具有极大的推广应用价值。Objective To analyze curative effect of high dose ambroxol hydrochloride combined with fiberoptic bronchoscope sputum suction in treatment of severe pneumonia. Methods 60 patients with severe pneumonia who were admitted to ICU of our hospital were selected as research objects and they were allocated to the research group and the control group randomly, with 30 in each. The research group was received treatment of high dose ambroxol hydrochloride combined with fiberoptic bronchoscope sputum suction at basis of routine treatment. The control group was received routine dose ambroxol hydrochloride combined with sputum-excretion treatment of vibratory sputumexcretion instrument. Clinical curative effects of patients in two groups were compared. Results After treatment, blood gas indexes including arterial oxygen pressure（Pa O2）, arterial oxygen saturation（Sa O2）, oxygenation index（Pa O2/Fi O2） and so on were all significantly improved and these blood gas indexes of the research group were significantly higher than those of the control group（P〈0.05）. Inflammatory indexes including white blood cell（WBC）, C-reactive Protein（CRP）, blood procalcitonin（PCT） and so on were all significantly decreased and these inflammatory indexes of the research group were all significantly lower than those of the control group（P〈0.05）. Accepted mechanical ventilation time of the research group was also significantly lower than that of the control group（P〈0.05）. Conclusion High dose ambroxol hydrochloride combined with fiberoptic bronchoscope sputum suction in treatment of patients with severe pneumonia has significant curative effect and high safety and it can shorten mechanical ventilation time which is beneficial for prognosis of patients. It has great value of promotion and application.
