摘 要:目的探讨3.0T磁共振3D-FIESTA及3D-TOF-MRA对血管压迫性面肌痉挛的诊断价值。方法选择25例临床诊断为面肌痉挛的患者,均进行3D-FIESTA和3D-TOF-MRA成像,并采用多平面重建技术观察面神经与邻近血管的关系,分析其与临床症状的相关性。结果 25例患者中,症状侧面神经存在血管接触或压迫21例,非症状侧5例,二者具有统计学差异(P<0.05),说明面肌痉挛发病与血管接触或压迫面神经密切相关。结论 3D-FIESTA结合3D-TOF-MRA可清晰显示面神经与毗邻血管的关系,对血管压迫性面肌痉挛的病因诊断有重要价值,可作为面肌痉挛患者的首选检查方法。Objective To explore the effectiveness of application of 3.0T MR 3D-FIESTA(Three Dimension-Fast Imaging Employing Steady State Acquisition) and 3D-TOF-MRA(Three Dimensionin diagnosis of neurovascular compression of HFS(Hemifacial Spasm). Methods Altogether 25 cases of patients with HFS were selected and scanned by using 3D-FIESTA and 3D-TOF-MRA. Multiplanar reformation was used to observe the relationship between the facial nerve and surrounding vessels so as to evaluate their relevance with clinical symptoms. Results 3D-FIESTA and 3D-TOFMRA showed facial nerve vascular compression or contact in 21 cases on the symptomatic sides and 5 cases on the asymptomatic sides among 25 patients, with statistically significant differences(P〈0.05), indicating that the clinical symptoms of hemi-facial spasm were closely associated with vascular compression. Conclusion As a preferred method for diagnosis of hemi-facial spasm, the application of the combination of 3D-FIESTA and 3D-TOF-MRA could clearly display the relationship between the facial nerve and surrounding vessels, which played an important role in etiological diagnosis of hemifacial spasm.
