摘 要:目的对比分析超声多普勒监测和生物物理相评分两种方法诊断胎儿宫内窘迫的临床意义。方法选取2012年1月至2014年12月成都市妇女儿童中心医院住院的55例临床怀疑胎儿宫内窘迫孕妇作为观察组,另选同期分娩的60例正常待产孕妇作为对照组。分别对两组孕妇进行超声多普勒监测和生物物理相评分,超声多普勒监测包括大脑中动脉(middle cerebral artery,MCA)、脐动脉(umbilical artery,UA)、静脉导管(ductus venosus,DV)的相关参数。出生后进行对比分析各项指标值及预测胎儿宫内窘迫的阳性率。结果观察组MCA的搏动指数(pulsatility index,PI)、阻力指数(resistance index,RI)、收缩期与舒张期流速比值(S/D)均低于对照组(P<0.05),UA的PI、RI、S/D值均高于对照组(P<0.05),DV的静脉搏动指数(pulsatility index for veins,PIV)、静脉峰值流速指数(peak velocity index vein,PVIV)均高于对照组(P<0.05)。观察组超声多普勒监测及生物物理相评分预测宫内窘迫的阳性率高于对照组,且超声多普勒监测3项联合生物物理相评分阳性率均高于单独预测的阳性率(P<0.05)。结论联合应用多普勒监测及生物物理相评分对诊断胎儿宫内窘迫有重要价值。Objective To analyze the clinical significance of doppler monitoring and biophysical score in diagnosis of fetal distress. Methods A total of 55 cases with clinical suspicion of fetal distress in Chengdu Wemen's and Children's Central Hospital from January 2012 to December 2014 were selected as observation group,60 cases of normal prepartal women in the same period were selected as control group. Two groups of pregnant women were conducted ultrasonic doppler monitoring and biophysical score,ultrasonic doppler monitoring included parameters of middle cerebral artery( MCA),umbilical arterial( UA) and ductus venosus( DV).The values of indexes and positive rates of predicting fetal distress were compared after birth. Results The pulsatility index( PI),resistance index( RI),S / D of MCA in observation group were lower than those in control group(P〈0. 05),the PI,RI,S / D of UA were higher than those in control group(P〈0. 05),the pulsatility index for veins( PIV),peak velocity index vein( PVIV) of DV in observation group were higher than those in control group(P〈0. 05). The positeve rate of predicting fetal distress with ultrasonic doppler monitoring and biophysical score in observation group were higher than control group,and the positeve rate of predicting fetal distress with 3 indexes of ultrasonic doppler joint with biophysical score were significantly higher than single item(P〈0. 05).Conclusion The ultrasonic doppler monitoring and biophysical score has an important value in diagnosis of fetal distress.
