我院静脉药物配置中心退药情况的分析与讨论

摘　　要：目的 对我院静脉药物配置中心的退药情况进行统计和分析,促进临床合理用药。方法 对我院静脉药物配置中心2014月7月~2015年7月的退药记录进行统计和分析,并结合退药单分析退药原因。结果 我院静脉药物配置中心2014月7月~2015年7月退药金额为1 787 195.51元,退药金额最多的依次为抗肿瘤药、营养类药物、抗生素、水电解质平衡药物、抗肿瘤药辅助用药和中药注射剂,退药原因主要有患者拒绝执行、不良反应、出院、手术取消、医嘱错误等。结论 退药分析反映了临床用药过程中的不足,静脉药物配置中心应规范管理和退药流程,医生应在药师指导和监督下合理用药,严格管理抗肿瘤药、抗生素、营养类药物等应用,加强与患者的沟通,提供个体化服务,从而减少退药。Objective To analyze the drug withdrawal in pharmacy intravenous admixture services （PIVAS） and to promote rational drug use. Methods The data of drug withdrawal between July 2014 and July 2015 in PIVAS were calculated and analyzed. And the drug withdraw notes were collected to analyze the causes. Results The snm of the drug retreated was ￥1 787 195.51. Among the retreated drugs, antineoplastic, nutrition support, antibiotic, drugs for water electrolyte balance and traditional Chinese medicine injection were the most frequent species. The causes for drug withdrawal included refusal of patients, adverse drug reac- tion, discharge, cancellation of surgery, irrational orders, and so on. Conclusion We should reduce drug withdrawal and promote the rational drug use by optimizing the process of PIVAS drug withdrawal, strengthening the study of incompatibilily of drugs in prescription, reducing antibiotics abuse, enhancing the communication with patients, and so on.
