无创机械通气联合雾化吸入治疗慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭18例及护理体会

摘　　要：目的观察综合护理干预在无创机械通气联合雾化吸入治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床疗效,并总结护理体会。方法选取2013年1月至2015年2月医院收治的合并Ⅱ型呼吸衰竭患者36例,均为老年患者,给予无创机械通气,并给予气道雾化吸入治疗。按护理方式不同分为观察组和对照组,各18例。观察组给予综合护理干预,对照组给予常规护理。结果观察组氧分压(PaO_2)、二氧化碳分压(PaCO_2)高于对照组而血氧饱和度(SpO_2)水平低于对照组(P<0.05);观察组呼吸机相关肺炎(VAP)发生率低于对照组(P<0.05);观察组出院满意度高于对照组(P<0.05)。结论无创序贯机械通气联合雾化吸入治疗COPD合并Ⅱ型呼吸衰竭疗效明显,实行综合护理干预可提高SpO_2,降低PaCO_2和VAP的发生率,提高出院满意度。Objective To investigate the clinical efficacy of comprehensive nursing intervention in the treatment of chronic obstructive pulmonary disease（COPD） complicated with type Ⅱ respiratory failure and the nursing experience.Methods 36 cases of elderly patients with COPD complicated with respiratory failure from January 2013 to Feb 2015 were selected.All patients were treated with non-invasive mechanical ventilation and airway atomization inhalation.According to the nursing methods,the patients were divided into the observation group and the control group,18 cases in each group.The control group was given routine nursing,which the observation group was given comprehensive nursing.Results The levels of PaO2 and PaCO2 were higher in the observation group than in the control group,but the level of SpO2 in the observation group was lower than in the control group（P〈0.05）;the incidence rate of VAP in the observation group was lower than that in the control group（P〈0.05）;the nursing satisfaction rate of the observation group was higher than that of the control group（P〈0.05）.Conclusion Non- invasive mechanical ventilation combined with atomization inhalation is an effective method for the treatment of COPD complicated with type Ⅱ respiratory failure,while the comprehensive nursing intervention can improve the level of SpO2,reduce the incidence rate of VAP and PaCO2,and improve the nursing satisfaction rate when discharge.
