阴式子宫切除术与腹腔镜辅助阴式子宫切除术的临床效果比较

摘　　要：目的比较阴式子宫切除术与腹腔镜辅助阴式子宫切除术的临床效果。方法选取2014年1月-2015年1月在本院接受子宫切除术治疗的80例患者作为研究对象,按照不同治疗方式分为对照组和研究组,各40例。对照组给予腹腔镜辅助阴式子宫切除术治疗,研究组给予阴式子宫切除术治疗,比较两组的治疗效果。结果研究组的总有效率显著高于对照组,差异有统计学意义（P〈0.05）。研究组的手术时间、首次排气时间、住院时间短于对照组,差异有统计学意义（P〈0.05）。研究组的下地活动时间显著短于对照组,差异有统计学意义（P〈0.01）。研究组的出血量显著少于对照组,差异有统计学意义（P〈0.01）。研究组的医疗费用显著少于对照组,差异有统计学意义（P〈0.01）。研究组的并发症发生率显著低于对照组,差异有统计学意义（P〈0.05）。结论阴式子宫切除术与腹腔镜辅助阴式子宫切除术均属于微创手术,但是阴式子宫切除术的治疗效果优于腹腔镜辅助阴式子宫切除术,可降低并发症发生率,促进身体恢复,减轻患者医疗负担,值得临床推广应用。Objective To compare the clinical effect of transvaginal hysterectomy and laparoscopic assisted vaginal hysterectomy. Methods 80 hysterectomy patients from January 2014 to January 2015 in our hospital were selected and divided into the control group and the study group according to different treatment methods,40 cases in each group.The control group was treated with laparoscopic assisted vaginal hysterectomy,the study group was treated with transvaginal hysterectomy.The therapy effect was compared between two groups. Results The clinical total effective rate in the study group was 92.5%,which was higher than 77.5% in the control group,with significant difference（P〈0.05）.Operation time,exhaust for the first time,hospitalization time in the study group was lower than that in the control group,with significant difference（P〈0.05）.Off-bed ambulation time in the study group was lower than that in the control group,with significant difference（P〈0.01）.The amount of bleeding in the study group was less than that in the control group,with significant difference（P〈0.01）.Hospitalization cost in the study group was less than that in the control group,with significant difference（P〈0.01）.The incidence rate of complication in the study group was lower than that in the control group,with significant difference（P〈0.05）. Conclusion Transvaginal hysterectomy and laparoscopic assisted vaginal hysterectomy are belong to the type of minimally invasive surgery,but the therapy effect of TVH is better than that LAVH,which can decrease the incidence rate of complication,promote the body recover and reduce the financial burden on patients medical,it is worthy of clinical promotion and application.
