摘  要:目的探讨超声引导下经锁骨上路行臂丛神经阻滞的效果及安全性。方法选取本院2014年11月-2015年10月的60例择期行上肢手术患者作为研究对象,随机分为A组和B组,各30例。A组采用超声引导下行锁骨上路臂丛神经阻滞,B组采用传统寻找异感法肌间沟臂丛神经阻滞。比较两组的肌皮神经、正中神经、桡神经、前臂内侧皮神经、尺神经阻滞完善率以及麻醉效果,同时记录相关并发症。结果 A组的正中神经、前臂内侧皮神经及尺神经阻滞完善率显著高于B组,差异有统计学意义(P〈0.01)。B组的肌皮神经及桡神经阻滞完善率显著高于前臂内侧皮神经及尺神经阻滞完善率,差异有统计学意义(P〈0.05)。A组的麻醉优良率显著高于B组,差异有统计学意义(P〈0.05)。A组未出现并发症,B组有3例并发症。结论超声引导下经锁骨上路行臂丛阻滞效果良好,并发症少。Objective To explore the effect and safety of brachial plexus block with the guidance of ultrasound guided by supraclavicular route. Methods 60 patients scheduled for upper limb surgery in our hospital form November 2014 to October 2015 were selected and randomly divided into group A and group B,30 cases in each group.Group A was given brachial plexus block with the guidance of ultrasound guided by supraclavicular route,and group B was given muscle sulcus brachial plexus block of finding paresthesia method.The block perfection rate of musculocutaneous nerve,median nerve,radial nerve,forearm medial cutaneous nerve,ulnar nerve and anesthetic effect in two groups was compared,and related complication was recorded. Results The block perfection rate of median nerve,forearm medial cutaneous nerve,ulnar nerve in group A was higher than that in group B,with significant difference(P〈0.01).The block perfection rate of musculocutaneous nerve and radial nerve in group B was higher than that of forearm medial cutaneous nerve and ulnar nerve,with significant difference(P〈0.05).The excellent and good rate of anesthesia in group A was higher than that in group B,with significant difference(P〈0.05). Conclusion The effect of brachial plexus block with the guidance of ultrasound guided by supraclavicular route is good,and complication is little.
