摘　　要：目的 探究2型糖尿病（T2DM）合并非酒精性脂肪肝（NAFLD）患者胰岛素抵抗的特点。方法 选取我院2014年7月~2015年9月收治的230例2型糖尿病患者,将其随机分成观察组（合并NAFLD）和对照组（非合并NAFLD）,各115例,比较两组患者各项生化指标、颈动脉内膜中层厚度（IMT）。结果 两组空腹血糖（FPG）、餐后2 h血糖（2h PG）和餐后2 h胰岛素（2 h INS）、糖化血红蛋白（Hb Alc）、空腹胰岛素（FINS）、胰岛素敏感指数（ISI）和胰岛素抵抗指数（HOMA—IR）、颈动脉内膜中层厚度（IMT）进行比较,差异均有统计学的意义,P〈0.05。结论 2型糖尿病合并非酒精性脂肪肝患者的血脂代谢紊乱,存在血糖升高、超重等特点。Objective To investigate the characteristics of insulin resistance in type 2 diabetes mellitus（T2DM）combined with nonalcoholic fatty liver disease（NAFLD）. Methods 230 patients with type 2 diabetes who were treated in our hospital from July 2014 to September 2015,were randomly divided into observation group（NAFLD）and control group（non NAFLD）,115 cases in each group,compared with two groups of patients with biochemical parameters,carotid intima media thickness（IMT）. Results Two groups of fasting blood glucose（FPG）,postprandial 2 h blood glucose（2 h PG） and 2 h insulin（2 h INS）,glycosylated hemoglobin（Hb Alc）,fasting insulin（FINS）,insulin sensitivity index（ISI）and insulin resistance index（IR,HOMA）,carotid intima media thickness（IMT）,the difference was statistically significant,P 0.05. Conclusion The metabolic disorder of lipid metabolism exists in type 2 diabetic patients with nonalcoholic fatty liver disease,and the characteristics of hyperglycemia,overweight and so on.
