可吸收缝线治疗急性髌骨脱位时骨软骨骨折的疗效

摘　　要：目的观察采用可吸收缝线治疗急性髌骨脱位时骨软骨骨折的疗效。方法从2010年1月至2014年12月,我科共治疗伴有骨软骨损伤的髌骨脱位患者15例。术中复位骨软骨骨折块后采用可吸收缝线固定,同时松解髌骨外侧支持带,修复或重建撕裂的内侧髌股韧带。术后进行Lysholm和IKDC膝关节功能评分,影像学观察骨软骨骨折修复情况。结果 15例患者术后恢复良好,无感染、再脱位等并发症。Lysholm评分由术前平均(38.5±6.8)分提高至末次随访的(80.4±7.5)分,IKDC评分由术前平均(36.8±6.2)分提高至末次随访的(76.2±5.9)分,差异有统计学意义(P<0.01)。术后影像学检查显示骨软骨骨折处愈合良好,无明显关节退变。结论采用可吸收缝线固定髌骨脱位时的骨软骨骨折,具有操作简单、无需二次取出内固定的优点。Objective To observe the effect of absorbable sutures in treatment of patellar dislocation complicated by os- teochondral fracture. Methods From January 2010 to December 2014,15 patients of patellar dislocation complicated by osteo- chondral fracture were treated in our department. Fractures were reduced and fixed with absorbable sutures ,lateral patellar reti- nacular were released, the medial patellofemoral ligament were repaired or reconstructed. The Lysholm and IKDC knee scoring system were used to evaluate the knee function,imaging examinations were used to observe the fracture healing. Results Pa- tients recovered well after operation without complications such as infection and redislocation. The Lysholm score improved from （ 38.5 ± 6.8 ） preoperative to（ 80.4 ± 7.5 ） at the last follow-up, and the IKDC score improved from（ 36.8 ± 6.2 ） preoperative to （76.2± 5.9 ） at the last follow-up, the difference was significant. Imaging revealed oteochondral fracture healed well. Conclu- sion Efficacy of absorbable sutures to treat patellar dislocation complicated by osteochondral fracture has the advantage of easy operation and no need of removing internal fixator.
