2002-2013年云南省实施结核病防治成本-效果与效益分析

摘　　要：目的分析云南省结核病防治成本-效果与效益。方法利用2002-2009年云南省结核病防治规划终期评估数据、2010-2013年结核病网络专报系统资料和全省各级经费报表,用Excel 2007进行数据统计分析。结果 2002-2013年间,全省共投入34 168.91万元,共登记肺结核患者249 099例,成功治疗214 923例,登记患者因结核病死亡1 509例,估算减少肺结核患者死亡60 762例,避免1 055 032名健康人感染肺结核,减少产生新的肺结核患者105 503例,挽回伤残调整生命年(DALY)1 405 845.73个,挽回社会经济价值1 487 652.56万元。在结核病防治工作上每投入1元,能产生43.75元的经济效益,每挽回1个DALY损失,政府仅需投入243.05元。结论通过结核防治规划达到了控制传染源、减少发病和死亡,减轻了患者的经济负担,减少了大量社会劳动力的损失,取得了显著的社会效益和经济效益。Objective To analyze the cost-effectiveness and cost-benefit of Tuberculosis（ TB） control in Yunnan Province. Methods The data collected from Final Assessment Report of TB Prevention Plan,online-based report system from2002 to 2013 and the financial statements of different district levels were analyzed in the study. Results A total of RMB 341 689100 had been invested in TB control during 2002 to 2013. There were 249,099 tuberculosis cases registered in this period and214 923 cases obtained complete treatment. Among registered cases,1,509 were died from TB. It was estimated that TB control in Yunnan Province reduced 60 762 death cases,avoided 1 055 032 healthy people infection and decreased 105 503 new cases.Total 1 405 845. 73 disability-adjusted life years（ DALY） were retrieved and RMB 14 876 525,600 were saved. The investment of every 1 Chinese yuan could win RMB 43. 75 in disease control. And the investment of RMB 243. 05 by government could redeem one DALY. Conclusion The TB prevention and treatment plan achieved the goals of infectious source control,incidence and mortality reduction and financial burden alleviation. Meanwhile,the implement of the program also reduced labor loss and achieved social and economic benefits.
