腰椎棘突偏歪征与歼击机飞行员腰椎间盘突出症预后及飞行结论的相关性分析

摘　　要：目的为了明确腰椎棘突偏歪征与飞行能力恢复状况的相关性,探讨其作为确定飞行结论参考指标的可行性。方法回顾2006年9月—2013年4月因腰椎间盘突出症在空军总医院中西医结合正骨治疗科住院治疗的31例歼击机飞行员诊治资料,确定治疗前后腰椎健康等级,并对其中医学鉴定为飞行合格（13例）和暂时飞行不合格（7例）的飞行员进行随访,通过χ^2检验分析治疗后腰椎健康等级与飞行能力恢复状况的相关性。结果飞行结论中飞行合格组与飞行不合格组腰椎健康等级的构成差异有统计学意义（χ^2=24.000,P=0.00）,随访结果中飞行合格组与飞行不合格组腰椎健康等级的构成差异有统计学意义（χ^2=-15.000,P=0.00）。结论根据腰椎棘突偏歪征制定的腰椎健康等级与腰椎间盘突出症歼击机飞行员的飞行能力恢复状况具有相关性,有助于确定飞行结论。Objective To clarify the correlation of deviation of the spinous process and flight performance restoring of fighter aviators with lumbar intervertebral disc herniation through a retrospective analysis of patients＇ data,and to discuss its feasibility as reference index for evaluation of fighter aviators.Methods This study reviewed medical records of 31 fighter aviators who were in hospital for lumbar intervertebral disc herniation in the Department of TCM Manipulative Orthopedics of Air Force General Hospital from September 2006 to April 2013.Their grading of health condition of low back pre-treatment and post-treatment was confirmed respectively.Follow-up visit was performed in 20 fighter aviators identified with suitable or temporarily unsuitable for flight when leaving hospital.Correlation between grading of health condition of low back post-treatment and flight performance restoring was analyzed with crosstabs-statistics.Results There was significant difference in the grading of health condition of low back between groups of suitable and unsuitable for flight（χ^2=24.000,P=0.00）.Significant difference in the grading of health condition of low back was also found in the groups of returning to flying and stopping flying through follow-up visit（χ^2=-15.000,P=0.00）.Conclusion There was correlation between flight performance restoring and the grading of health condition of low back based on the deviation of the spinous process.Therefore,the grading is helpful for the evaluation of fighter aviators with lumbar intervertebral disc herniation.
