摘  要:目的观察通窍活血汤对脑外伤大鼠认知功能的影响并探索其作用机制。方法 70只雄性Sprague-Dawley大鼠随机分为正常组(n=10)、假手术组(n=20)、自然恢复组(n=20)和治疗组(n=20)。采用控制性皮质撞击的方法制作大鼠脑外伤模型。治疗组大鼠在造模后每只鼠平均每天饮通窍活血汤5 ml,其他组每天饮用蒸馏水,持续4周。术后1、2、3、4周对大鼠进行Morris水迷宫测试,免疫组化检测海马区脑源性营养因子(BDNF)和突触素(SynⅠ)的表达。结果自然恢复组各时间点逃避潜伏期较正常组增加(P<0.05),治疗组1、2周较正常组增加(P<0.05),3、4周无显著性差异(P>0.05);治疗组3、4周小于自然恢复组(P<0.05)。自然恢复组各时间点游泳时间百分比较正常组降低(P<0.05);治疗组1、2、3周较正常组降低(P<0.05),4周时无显著性差异(P>0.05);治疗组4周时游泳时间百分比高于自然恢复组(P<0.05)。自然恢复组1、2周时海马区BDNF表达低于正常组(P<0.05),3、4周时无显著性差异(P>0.05),治疗组1、2、3周与正常组无显著性差异(P>0.05),4周时明显高于正常组(P<0.01);治疗组在2、3、4周海马区BDNF表达高于自然恢复组(P<0.05)。自然恢复组各时间点SynⅠ表达显著低于正常组(P<0.001),治疗组1、2、3周明显低于正常组(P<0.01),4周时无显著性差异(P>0.05);治疗组3、4周明显高于自然恢复组(P<0.01)。结论通窍活血汤能够改善脑外伤大鼠的认知功能,其机制可能是通过影响海马区的神经重塑。Objective To observe the effect of Tongqiaohuoxue decoction on cognitive impairment in rats with traumatic brain injury and explore the mechanisms. Methods 70 Sprague-Dawley rats were randomized into normal group(n=10), sham operated group(n=20),untreated group(n=20) and treatment group(n=20). Controlled cortical impact device was utilized to establish traumatic brain injury model.The treatment group received Tongqiaohuoxue decoction 5 ml/d, and other groups received distilled water. The cognitive function of rats was evaluated with Morris water maze 1, 2, 3, 4 weeks post operation. And the rats were sampled to test the expression of brain-derived neurotrophic factor(BDNF) and synaptophysin(Syn) I in hippocampus through immunohistochemistry. Results The escape latency was longer in the untreated group than in the normal group in all the time points(P〈0.05), and was longer in the treatment group than in the normal group 1 and 2 weeks after injury(P〈0.05), with no significant difference 3 and 4 weeks after injury(P〉0.05). And it was shorter in the treatment group than in the untreated group 3 and 4 weeks after injury(P〈0.05). The percentage of swimming time was lower in the untreated group than in the normal group in all the time points(P〈0.05), and was lower in the treatment group than in the normal group 1, 2, and 3weeks after injury(P〈0.05), with no significant difference 4 weeks after injury(P〉0.05). And it was higher in the treatment group than in the untreated group weeks after injury(P〈0.05). The BDNF expression was lower in the untreated group than in the normal group 1 and 2weeks after injury(P〈0.05), with no significant difference 3 and 4 weeks after injury(P〈0.05). It was higher in the treatment group than in the normal group 4 weeks after injury(P〈0.01), and was higher in the treatment group than in the untreated group 2, 3 and 4 weeks after injury(P〈0.05).
