摘  要:目的采用基于体素的节点中心度(DC)探讨轻型脑外伤(MTBI)患者功能脑网络组体素水平节点单元拓扑属性的变化特点。方法对13例轻型脑外伤患者(MTBI组)及与之年龄、性别相匹配的15例健康志愿者(正常对照组)行静息态功能磁共振扫描(R-f MRI)及临床相关量表评分,使用SPM 8及DPARSFA在Matlab 2012b平台上采用基于体素的DC(相关系数r阈值为0.25,二值化连边矩阵)方法分析R-f MRI数据;采用卡方检验及t检验比较组间参数,对MTBI组差异脑区的DC值分别与临床参数进行相关性检验。结果与正常对照组相比,MTBI组右侧额上回DC值增高;右侧颞中回、双侧顶下小叶、左侧小脑半球DC值显著减低(P<0.05,GRF多重校正)。右侧额上回的DC值与运动功能评估量表(r=0.77)正相关,与格拉斯哥昏迷量表(r=-0.75)、日常生活能力量表(r=-0.76)负相关。左侧顶下小叶(缘上回)DC值与简易智力量表(r=0.64)正相关。结论 MTBI患者DC的改变及其与临床参数的关联,提示其神经认知功能改变的功能脑网络基础。Objective Using voxel wise degree centrality ( DC), we aimed to investigate the functional brain network group of voxel unit level node topological properties change characteristics in patients with mild traumatic brain injury ( MT- BI). Methods Thirteen MTBI patients (MTBI group) and 15 age and gender matched healthy controls (Control group) underwent a resting state functional magnetic resonance imaging ( RfMRI), and clinical scales were also collected. Data of RfMRI were processed on Matlab 2012b station by using degree centrality method ( threshold value 0.25 ), and interclass differences were analyzed. Correlation analysis was performed for regions with DC value statistically different in MTBI group with clinical parameters. Results Compared with control group, significantly increased DC was found in right superior frontal gyms, as well as significantly decreased DC was found in right middle temporal gyms, bilateral Inferior parietal lob- ule, left cerebellum in MTBI group (P 〈 0.05, GRF corrected). DC value in right superior frontal gyms positively correla- ted with Motor Assessment Scale (r -- 0.77), and negatively correlated with Glasgow Coma Scale ( r = - 0.75) and Activi- ty of Daily Living Scale ( r = - 0.76). Left Inferior parietal lobule positively correlated with Mini Mental State Examination (r = 0.64). Conclusion Differences of DC value in MTBI group, which is correlated with clinical parameters, suggest that the brain network foundation of the neurocognitive functions change.
