原发性失眠默认网络亚区功能连接的功能磁共振成像研究

摘　　要：目的应用静息态功能连接方法探讨原发性失眠患者(PIs)默认网络(DMN)内各亚区的功能连接变化。方法 42例PIs及与之年龄、性别匹配的42例正常睡眠志愿者(GSs)均接受功能磁共振成像扫描,应用种子点功能连接方法及SPM8软件包进行数据处理,同时分析临床量表与DMN各成对亚区功能连接的相关性。结果 PIs组与GSs组的匹兹堡睡眠质量指数量表(PSQI)、汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)、睡眠状况自评量表(SRSS)、心境状态量表(POMS)差异均具有统计学意义(P<0.05)。与GSs组相比,PIs组内侧前额叶(MPFC)与右侧内侧颞叶(RMTL)(t=-2.275,P=0.026)的功能连接减低;左侧内侧颞叶(LMTL)与左侧顶下皮质(LIPC)(t=-3.32,P=0.001)的功能连接减低。结论 PIs组存在DMN内亚区功能连接异常,MPFC-RMTL、LMTL-LIPL的功能连接减低可能与PIs认知功能及情感障碍的神经机制有关。Objective To explore the resting state functional connectivity （FC） of the paired default mode network （DMN） sub-regions in patients with primary insomnia （PIs）. Methods Forty two PIs and forty two age, sex matched good sleepers （GSs） were recruited and underwent resting state functional magnetic resonance imaging scans. The Seed based region to region FC method and SPM8 software were used to evaluate whether paired DMN sub-regions was modulated between PIs and GSs, and the correlations between DMN sub-regions and clinical markers. Results Compared with GSs, PIs showed higher Pittsburgh Sleep Quality Index （PSQI） , Hamilton Anxiety Rating Scale （HAMA）, Hamilton Depression Rating Scale （HAMD） , Self-Rating Scale of Sleep （SRSS） , Profile of Mood States （POMS） scores （ P 〈 0.05）. PIs showed significant decreased region to region FC between the Medial prefrontal cortex （MPFC） and right Medial temporal lobe （RMTL） （ t = - 2. 275, P = 0.026 ） , and between left Medial temporal lobe （LMTL） and left Inferior parietal cortices （LIPC） （t = -3.32, P = 0.001 ）. Conclusion PIs showed disrupted FC within the DMN sub-regions, which may pro- vide information for the neural mechanisms underlying cognitive and emotion impairment of PIs.
