摘  要:目的探讨Vibrant动态增强联合MR血管成像在乳腺良恶性病变诊断中应用价值。方法36例患者共40个病灶,其中恶性病灶18个,良性病灶22个,行乳腺动态增强MRI检查,并重建得到3D—MIP图像。在GEADW4.3图像工作站运用Functool功能软件对增强病变进行血流动力学分析。结果以Ⅱ-Ⅲ型曲线,信号增强率〉90%,峰值时间出现在3min之前,周围血管征阳性作为恶性标准,敏感性分别为88.9%、88.9%、83.3%、77.8%,特异性分别为72.7%、54.5%、90.9%、86.4%,准确性分别为80%、70%、87.5%、82.5%,阳性预测值分别为72.7%、61.5%、88,2%、82.4%,阴性预测值分别为88.9%、85.7%、86.9%、82.6%,良恶性差异有统计学意义(P〈0.05)。结论Vibrant动态增强血流动力学特征联合周围血管征在乳腺良恶性病变判断方面具有重要价值。Objective To evaluate the diagnostic value of vibrant magnetic resonance dynamic contrast enhanced and adjacent vessel sign in the diagnosis of benign and malignant breast Lesions. Methods Total 36 patients with 40 breast lesions including 18 malignant and 22 benign ones, underwent breast dynamic contrast enhanced MR scan. The 3D maxi- mum intensity projection images were obtained. All imaging were transferred to functional work station and evaluated the dynamics of enhanced lesions at GE ADW 4.3. Results For type Ⅲ curve, signal enhancement ratio (SER) 〉 90% af- ter one minute, time to peak (TrP) 〈3 min, and adjacent vessel sign as the malignant criteria, the sensitivity (88.9% , 88.9% , 83.3% , 77.8% ) , specificity ( 72.7% , 54.5% , 90.9% , 86.4% ) , accuracy rate ( 80% , 70% , 87.5% , 82. 5% ) ,predictive value positive (72.7% ,61.5% ,88,2% ,82.4% ) , predictive value of negative were( 88.9% ,85.7% , 86.9% ,82.6% )respectively. There was statistical significance between benign and malignant lesions. Conclusion Vi- brant dynamic contrast enhanced curve and adjacent vessel sign is great importance in differentiation of benign and malig- nant lesions.
