基质金属蛋白酶-2及其组织抑制因子-1在子宫内膜异位症中的表达及意义

摘　　要：目的:探讨基质金属蛋白酶-2(matrix matalloproteinase-2,MMP-2)及组织抑制因子-1(tissue inhibitor of metalloproteinase-1,TIMP-1)在子宫内膜异位症发生发展中的作用。方法:选择子宫内膜异位症患者异位内膜组织(卵巢巧克力囊肿的囊壁组织)30例、在位内膜组织22例和对照组正常子宫内膜组织20例为研究对象。采用免疫组化法检测3组内膜组织中MMP-2、TIMP-1的表达。结果:MMP-2在正常子宫内膜腺上皮细胞、异位和在位内膜的上皮及间质均表达,在异位内膜组织中的表达高于在位内膜和正常内膜组织,差异有统计学意义(P<0.05);在位内膜组织中MMP-2的表达高于正常内膜组织,差异有统计学意义(P<0.05)。TIMP-1在正常子宫内膜腺上皮细胞、异位和在位内膜的上皮及间质均表达,其在异位内膜和在位内膜组织中的表达低于正常子宫内膜,差异有统计学意义(P<0.01),异位内膜与在位内膜TIMP-1的表达差异无统计学意义(P>0.05)。结论:异位和在位内膜组织中MMP-2高表达、TIMP-1的低表达,导致基质金属蛋白酶/组织抑制因子的比例增高,使子宫内膜异位症患者的异位内膜侵袭性更强,在该病的发生、发展中起着极其重要的作用。Objective： To evaluate the role of the matrix matalloproteinase- 2（ MMP- 2） and tissue inhibitor of metalloproteinase- 1（ TIMP- 1） in the pathogenesis of endometriosis. Method： Immunohistochemical SABC method was employed to detect the expression of MMP- 2 and TIMP- 1 in ectopic endometriosis tissue（ ovarian chocolate cyst tissue of cystiform）（ 30 cases）,eutopic endometrium（ 22 cases） from women with endometriosis and the normal controls（ 20 cases）. Results： The expressions of MMP- 2 were detected in the normal epithelial glandular cells and endometrial glandular cells and stromal cells in ectopic and eutopic endometrium. The expression of MMP- 2 in ectopic endometrial tissues was significantly higher than those in the eutopic and normal endometrial tissues（ P〈 0. 05）. The expression of MMP- 2 in eutopic endometrial tissues was significantly higher than that in the normal endometrial tissues（ P 〈0. 05）. The expressions of TIMP- 1 were detected in the normal epithelial glandular cells and endometrial glandular cells and stromal cells in ectopic and eutopic endometrial tissues. The expression of TIMP- 1 in ectopic and eutopic endometrial tissues was significantly lower than that in the normal endometrial tissues（ P 〈0. 05）. And there was no significant difference between the ectopic and eutopic endometrial tissues from women with endometriosis. Conclusions：The greater expression of MMP- 2 and the lower expression of TIMP- 1 in the ectopic and eutopic endometrial tissues result in the higher ratio of MMPs / TIMPs,in which the ectopic endometrial tissues are more aggressive to the women with endometriosis.
