以发热为主要症状的21例亚急性甲状腺炎患者因误诊应用抗菌药物的分析

摘　　要：目的总结发热门诊21例亚急性甲状腺炎误诊、误治的主要原因,为减少抗菌药物不合理使用积累循症医学证据。方法回顾2010年12月至2014年12月北京世纪坛医院发热门诊21例伴有发热的亚急性甲状腺炎患者的临床资料,总结并分析亚急性甲状腺炎误诊、误用抗菌药物的主要原因。结果伴有发热的亚急性甲状腺炎误诊患者均曾应用不同种类抗菌药物,多为中、青年,女性多于男性,临床症状主要是发热和咽痛,部分患者伴有下颌、耳部或颈部放射痛、合并一侧或双侧甲状腺肿大和出现甲状腺功能异常,21例患者均接受过不同种类的抗菌药物治疗,原因是11例误诊为咽炎,10例误诊为呼吸道感染。结论亚急性甲状腺炎常表现为咽喉部不适,由于发热门诊医生对该病缺乏足够认识,常误诊为呼吸道感染或咽炎而造成抗菌药物不合理使用。Objective To explore the main causes of misdiagnosis Of 21 subacute thyroiditis patients with fever as the main symptoms in order to accumulate medical evidence for reduction the irrational use of antibiotics. Methods The retrospective analysis of the clinical data of 21 subacute thyroiditis patients with fever from December of 2010 to December of 2014 in fever clinic were carried out to summarize and analyze the main reason of misdiagnosis of subacute thyroiditis and misuse of antimicrohial drugs. Results Misdiagnosed subacute thyroiditis patients were mostly in young or middle age female patients. The main clinical symptoms were fever with sore throat, some patients with mandible, ear or neck pain radiation and combined with unilateral or bilateral thyroid enlargement and appear abnormal thyroid function. All 21 patients received different type antimicrobial therapy. The reason was that 11 cases of patients were misdiagnosed as pharyngitis and 10 cases were misdiagnosed as respiratory tract infection. Conclusion Subacute thyroiditis often express the symptom of throat discomfort. The misdiagnosed of subacute thyroiditis as respiratory tract infection or pharyngitis might lead to the irrational use of antimicrobial drugs.
