轻度胃肠炎伴发良性婴幼儿惊厥血清钠与血浆渗透压变化的研究

摘　　要：目的分析轻度胃肠炎伴发良性婴幼儿惊厥（BICE）患儿起病至抽搐发作时间、抽搐发作次数、抽搐持续时间与血清钠及血浆渗透压水平变化情况。方法对2011年1月—2013年12月诊断为BICE患儿57例的临床资料进行回顾性分析。结果本组57例,男23例,女34例,年龄6~36个月,平均（18.5±6.5）个月。其中存在低钠血症（血钠〈130mmol/L）19例,血钠130~135mmol/L的24例,低渗透压（渗透压〈285mOsm/L）21例。起病至抽搐发作的时间〈24h的6例,24~48h的23例,〉48h的28例,平均2.4d。病程中的抽搐发作次数：发作1次42例,发作2次13例,发作3次2例。抽搐发作持续时间〈1min 23例,持续1~5min 29例,持续〉5min 5例。结论 BICE患儿的渗透压水平在不同的起病至抽搐发作时间及抽搐发作次数差异无统计学意义,而血钠水平差异有统计学意义。血钠、渗透压水平在不同的抽搐持续时间差异均有统计学意义。Objective To analyze the time of clinical manifestation before seizure onset,frequency and duration of the seizures’ occurrence,as well as the changes of serum Sodium and plasma osmotic pressure in children with benign infantile convulsions associated with mild gastroenteritis（BICE）.Methods A retrospective study was conducted of 57 children diagnosed with BICE between January 2011 to December 2013.Results Fifty-seven children with BICE（23 males and34females）were recruited.The age of onset ranged from6to36 months,average age was 18.5±6.5 months.In our study,there were 19 patients with hyponatremia（serum Sodium〈130mmol/L）,24 patients with normonatremia（serum Sodium belongs to130-135mmol/L）,21 patients with low osmolality（plasma osmotic pressure〈285mOsm/L）.We observed that the interval between the onset of gastroenteritis and that of seizures was less than24 hours in6 patients,between24 and48 hours in23 patients,and longer than48 hours in28 patients.The number of seizures during a single episode was 1in42 patients,2in13 patients,3in 2 patients.The duration of the seizures was less than1 minutes in23 patients,persisted from1to5 minutes in29 patients,persisted longer than5 minutes in5 patients.Conclusions There are no differences in children with BICE on plasma osmotic pressure between frequency and duration of the seizures,but significant differences on serum Sodium.There are remarkable differences on serum Sodium and plasma osmotic pressure at different durations of seizures.
