不同鼻饲喂养器具对机械通气重力鼻饲喂养效果的影响

摘　　要：目的探讨不同鼻饲喂养器具对机械通气重力鼻饲喂养效果的影响。方法将281例机械通气的患儿按不同年龄分为4组（3~6个月、7~12个月、13~36个月、37~60个月）,每个年龄组随机分为观察组（改良的重力鼻饲喂养组）和对照组（传统的重力鼻饲喂养组）,观察患儿喂养时有无呕吐、胃潴留、腹胀、呼吸机相关性肺炎的发生。结果观察组和对照组患儿呕吐、胃潴留、腹胀及合并呼吸机相关性肺炎比较,3~6个月、7~12个月、13~36个月年龄组差异有统计学意义,尤其在合并呼吸机相关性肺炎方面差异有统计学意义（P〈0.01）;而37~60个月观察组和对照组患儿在呕吐、胃潴留、腹胀、合并呼吸机相关性肺炎的比较差异无统计学意义（P〉0.05）。结论机械通气患儿在3~6个月、7~12个月、13~36个月年龄组应用改良重力鼻饲喂养耐受性好,营养物质的吸收好,机械通气并发症的发生减少,从而促进患儿康复,降低患儿家属的经济负担。Objective To study the effect of different nasogastric feeding apparatus on mechanical ventilation gravity nasogastric feeding.Methods A total of 281 cases of mechanical ventilation were divided into 4groups according to different age（3-6 months,7-12 months,13-36 months,37-60 months）,each group was randomly divided into experimental group（the gravity of the modified nasogastric feeding group）and control group（traditional gravity nasogastric feeding group）,the vomiting,gastric retention,abdominal distension,the occurrence of ventilator associated pneumonia were observed during children feed.Results The difference of experimental group and control group on vomiting,gastric retention,abdominal distension and combination of ventilator associated pneumonia in 4 age groups was statistically significant,especially in the aspect of combination of ventilator associated pneumonia has significant difference（P〈0.01）;While in the experimental group and the control group of 37-60 months children in vomiting,gastric retention,abdominal distension,merging,there was no statistically significant difference comparing the ventilator associated pneumonia（P〉0.05）.Conclusions For children with mechanical ventilation in different age groups,modified nasogastric feeding can improve toleration,increase the absorption of nutrients,reduce mechanical ventilation complications,and can promote children rehabilitation and reduce the economic burden of children’s family.
