泼尼松联合环磷酰胺治疗特发性肺纤维化临床疗效观察

摘　　要：目的观察泼尼松联合环磷酰胺治疗特发性肺纤维化的临床疗效。方法选取2012年3月至2014年12月在我院治疗的128例特发性肺纤维化患者为研究对象,随机分为对照组和观察组各64例。对照组予泼尼松治疗,观察组在对照组基础上予环磷酰胺治疗。观察两组患者入院时、第4周、第12周、第24周肺活量(VC)和动脉血氧分压(Pa O2)变化,以及治疗期间不良反应情况。结果观察组在第4周、第12周、第24周VC、Pa O2恢复程度均高于对照组,差异均有统计学意义(P<0.05);对照组总不良反应率为31.3%,观察组为34.4%,差异无统计学意义(P>0.05);对照组总有效率为75.0%,观察组为89.1%,观察组总有效率高于对照组,差异有统计学意义(P<0.05)。结论泼尼松联合环磷酰胺治疗特发性肺纤维化患者能有效改善患者肺活量及动脉血氧分压,安全有效。Objective To observe the clinical effect of prednisone combined with cyclophosphamide in the treatment of idiopathic pulmonary fibrosis. Methods A total of 128 patients with idiopathic pulmonary fibrosis in the hospital from Mar. 2012 to Dec. 2014 were selected as the research objects, which were randomly divided into control group and observation group, each with 64 cases. The control group was treated with prednisone, while the observation group received cyclophosphamide on the basis of the control group. The vital capacity（VC） and the changes of arterial blood oxygen partial pressure（Pa O2） were observed at admission, 4 weeks, 12 weeks and 24 weeks. The adverse reactions during the treatment were recorded. Results The recovery degrees of VC, Pa O2 at 4 weeks, 12 weeks and24 weeks were significantly higher in the observation group than the control group（P〈0.05）. The total rate of adverse reaction was 31.3% in the control group and 34.4% in the observation group, with no statistically significant difference（P〈0.05）. The total effective rate was 89.1% in the observation group, significantly higher than 75.0% in the control group（P〈0.05）. Conclusion Prednisone combined with cyclophosphamide can effectively improve the vital capacity and arterial blood oxygen partial pressure in patients with idiopathic pulmonary fibrosis, which is safe and effective.
