摘  要:目的通过分析首发冠心病患者的颈围和血脂谱,探讨血脂谱、颈围与冠心病发病及病变程度的相关性。方法选取2013年1月至2014年12月期间在惠州市中心人民医院经冠状动脉造影确诊的冠心病患者367例作为观察组,以同期体检健康的志愿者310例作为对照组,测量颈围,同时用全自动生化分析仪进行血脂及载脂蛋白分析。颈围与血脂谱相关性采用Pearson相关分析,采用Logistic回归分析冠心病的危险因素。结果 1总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白B(Apo B)、非高密度脂蛋白胆固醇(non-HDL-C)、TC/HDL-C、Apo B/Apo AⅠ和颈围等指标在观察组明显高于对照组,观察组的Apo AⅠ低于对照组,差异均有统计学意义(P<0.05);2除Apo B外,观察组中的血脂谱和颈围在不同的Gensini积分分组的组间比较,差异均有统计学意义(P<0.05);3颈围与TC、LDL-C、Apo B、TC/HDL-C、Apo B/Apo AⅠ、non-HDL-C呈正相关,与HDL-C、Apo AⅠ呈负相关(P<0.05)。4 Logistic分析显示,颈围、LDL-C为冠心病事件发生的独立危险因素(OR=1.128,95%CI:1.075,1.185;OR=2.872,95%CI:2.182,3.781)。结论颈围与血脂谱联合检测可作为预测冠心病事件发生及病变程度的简单、廉价、实用的方法。Objective To investigate the neck circumference(NC) and lipid profile in patients with coronary heart disease(CHD), and analyze the correlation of NC and lipid profile with CHD. Methods From January 2013 to December 2014, 367 patients of CHD confirmed by coronary angiography in Huizhou Municipal Central Hospital were enrolled as the observation group, and 310 healthy adults were selected as the control group. NC was measured,and the serum lipid and apolipoprotein in the two groups were detected by automatic biochemical detector. Pearson correlation analysis was applied to analyze the association between NC and lipid profile, and logistic regression analysis was used to analyze the risk factors for CHD. Results The values of total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), apolipoprotein B(Apo B), non-high density lipoprotein cholesterol(non-HDL-C), TC/HDL-C ratio, Apo B/Apo AⅠ ratio, and NC in the observation group were significantly higher than those in the control group, while the concentration of apolipoprotein AⅠ(Apo AⅠ) in the observation group was significantly lower(P〈0.05). In the observation group, the lipid profile and NC showed statistically significant difference between different Gensini score groups(P〈0.05), except for Apo B. Pearson correlation analysis revealed that NC was positively correlated with TC, LDL-C, Apo B, TC/HDL-C, Apo B/Apo A Ⅰ, non-HDL-C, but negatively correlated with HDL-C, Apo AⅠ. Logistic regression analysis indicated that NC and LDL-C were independent predictors for CHD(OR=1.128, 95%CI: 1.075, 1.185; OR=2.872, 95%CI: 2.182, 3.781). Conclusion Detection of NC and lipid profile can be a simple,inexpensive and practical way for predicting the occurrence and severity of CHD.
