锁定钢板内固定治疗青少年胫骨远端三平面骨折的临床疗效

摘　　要：目的 探讨锁定加压钢板（LCP）内固定治疗青少年胫骨远端三平面骨折的临床疗效。方法 自2012-06—2014-06采用LCP治疗106例青少年胫骨远端三平面骨折,随机分成观察组（采用LCP）和对照组（采用传统切开复位内固定）。结果 观察组手术时间（χ^2=5.321,P=0.000）、术中出血量（χ^2=12.960,P=0.000）与对照组比较,差异有统计学意义（P〈0.05）。2组住院时间（χ^2=0.218,P=0.828）及骨折愈合时间（χ^2=0.217,P=0.828）比较,差异无统计学意义（P〉0.05）。术后所有患者获得平均6（4-12）个月随访。2组临床治疗效果比较,观察组优37例,良16例,优良率100%,对照组优31例,良20例,差2例,优良率96.2%,2组优良率（χ^2=2.038,P=0.153）比较,差异无统计学意义（均P〉0.05）。观察组并发症发生率（χ^2=2.038,P=0.153）与对照组相比,差异无统计学意义（均P〉0.05）。结论 LCP治疗青少年胫骨远端三平面骨折,手术时间与术中出血量较少,具有较好的治疗效果,且术后相关并发症较少,值得临床推广应用。Objective To investigate the clinical efficacy of locking plate for triplane fractures of distal tibia in teenagers.Methods From June 2012 to June 2014, 106 teenagers with triplane fracture of the distal tibia were treated with locking plate. They were randomly divided into observation group（treated with locking plate） and control group（treated with conventional open reduction and internal fixation）. Results Between the observation group and the control group, in the operation time（χ^2=5.321, P =0.000）, and the blood loss in operation（χ^2=12.960, P =0.000）, there were statistically significant differences（P〈0.05）. However, in hospital stay（χ^2=0.218, P =0.828） and fracture healing time（χ^2=0.217, P =0.828）, there were no statistically significant differences between these two groups（P〉0.05）. All patients were followed up for average 6（4-12）months. The efficacy of two groups was compared. In the observation group, 37 cases were excellent, 16 cases good, the good and excellent rate was 100%. In the control group, 31 cases were excellent, 20 cases good, 2 cases bad, the good and excellent rate was 96.2%. There was no significant difference in the good and excellent rate（χ^2=2.038, P =0.153） between these two groups（P〉0.05）. There was no significant difference in the complication rate（χ^2=2.038, P =0.153） between these two groups（P〉0.05）. Conclusion Application of locking plate has the good therapeutic effect for triplane fractures of distal tibia in teenagers, with shorter operation time and less blood loss in operation and it also has fewer postoperative complications. It is worthy of clinical recommendation.
