摘　　要：目的探讨免疫性血小板减少症(ITP)中外周血Th17与Treg细胞免疫平衡的变化及临床意义。方法随机选取80例免疫性血小板减少症患者为研究对象,以37例为活动组,24例为显效组,13例为有效组,6例为未缓解组,另选同期体检健康者25例为对照组,对所有患者外周血中Th17细胞及Treg细胞在CD4^+T细胞比率进行检测。结果对照组外周血Th17细胞占CD4^+T细胞百分率均低于其他组,同活动组及未缓解组差异显著(P<0.05),同显效组及有效组差异不显著(P>0.05);对照组外周血Treg细胞占CD4^+T细胞百分率以及外周血Th17/Treg细胞比率同活动组、有效组及未缓解组差异显著(P<0.05),同显效组差异不明显(P>0.05)。结论在ITP患者中,Th17及Treg细胞会出现变化,变化程度同病情有关联,Th17/Treg免疫失衡在ITP发病机制中具有重要意义。Objective To study the changes and clinical significance of Th17 and Treg cells immune balance of peripheral blood in immune thrombocytopenia（ ITP）. Methods Randomlyselected 80 patients with ITP as the research object,in 37 cases,for group activities,24 cases of group had marked effect,13 cases for effective group,6 cases was no remission group,an alternate period check-up 25 cases of healthy subjects as control group,Th17 cells and Treg cells in peripheral blood in patients with all in the ratio of CD4~＋T cells were detected. Results The added data of Th17 cells of CD4~＋T cell percentage in control group was lower than other groups,had significant difference with activity and non-remission group（ P〈0. 05）,noobvious difference with marked effect and effective group（ P〈0. 05）; In control group,Treg cellspercentage of CD4~＋T cells and the ratio of Th17/Treg cells in peripheral blood had significant difference with activity、effective and non-remission group（ P〈0. 05）,no obvious difference with marked effect group（ P〈0. 05）. Conclusion In patients with ITP,there will be a change of Th17 and Treg cells,and the degree of change is closely associated with the illness degree,and Th17/Treg immune imbalance is of great significance in the pathogenesis of ITP.
