摘  要:目的探讨第三代海德堡视网膜断层扫描仪(HRT-3)检测视网膜神经纤维层(RNFL)厚度与可疑原发性开角型青光眼(POAG)病程进展的关系,评价HRT-3在早期青光眼诊断中的意义。方法用HRT-3对正常组20例(40只眼)及眼压增高的可疑POAG患者13例(24只眼)进行RNFL厚度扫描,并于6个月后对同一位置的RNFL厚度进行追踪扫描,统计并分析比较两次测得的结果。结果可疑POAG组在高眼压的状态下随访6个月后视盘周围RNFL厚度在上方(S)、下方(I)、颞上方(TS)、鼻上方(NS)、颞下方(TI)、鼻下方(NI)以及平均厚度(G)比初次就诊时明显变薄,差异有统计学意义(P<0.05)。结论随着可疑POAG患者病情的发展,RNFL厚度逐渐减小,HRT-3能够准确反映视盘周围RNFL厚度改变,较好的评价早期POAG病情进展情况,并能为临床早期诊断青光眼提供更多的信息。Objective Discuss the relationship between retinal nerve fiber layer thickness( measured by third generation of Heidelberg retinal tomography) and the progression of suspected primary open-angle glaucoma. To evaluate the application of HRT-3 in the diagnosis of early glaucoma. Methods Authors scanned the RNFL thickness with HRT-3 in20 normal subjects( 40 eyes) and 13 suspected POAG patients with high IOP. A follow-up scan of RNFL thickness at the same position was done in 6 months later. Results Peripapillary RNFL thickness in Suspected POAG patients over the superior( S),inferior( I),temporal superior( TS),nasal superior( NS),temporal inferior( TI),nasal inferior( NI) and the general thickness( G) became significantly thinner in six months after initial measurement( P〈0. 05). Conclusion RNFL thickness decreases in suspected POAG patients within 6 months of initial evaluation. Heidelberg retinal tomography( HRT-3) can accurately detect the changes in peripapillary RNFL thickness. It is an excellent tool to evaluate the progression of early POAG and to provide necessary information for the diagnosis.
