摘  要:目的探讨新生儿脐带血中CD4+CD25+CD127tow调节性T细胞和pDC频数和功能。方法收集2013年8月至2014年2月在北京地坛医院妇产科分娩的HBVDNA阴性(〈100IU/m1)孕妇所生新生儿脐带血以及正常成人外周静脉血,采用流式细胞仪分析技术测定两组全血样本中CD4+CD25+CD127lowTreg细胞占CD4+细胞比率以及pDC频数和表面分子水平;采用Luminex技术分别检测两组血清中细胞因子IFN—a、IFN-t、IL-6、IL-10、IL-17A、Fh-3L水平。结果新生儿脐带血CD4-I-CD25+CD127lowTreg细胞频数(7.34±1.54)与正常成人组(8.14±1.15)之间差异无统计学意义;新生儿Linl-HLA—DR+CDl23+pDC细胞频数及其表面功能分子CD80、CD83和CD86阳性率均显著低于正常成人组(P=0.000),而TLR-9表达阳性率(21.30±22.50)与成人(34.27±11.31)差异无统计学意义;新生儿脐血中细胞因子IFN-r及IL-17A水平显著低于正常成人组(P=0.003,P=0.010),而IFN-a、IL-10、IL-6及Flt-3L水平在两组之间差异无统计学意义。结论新生儿pDC频数、成熟度低于成人可能与新生儿感染HBV易慢性化有关。Objective The aim of this study was to study on the frequency and function of CD4 ± CD25 ± CD127low regulatory T cells and pDC from neonatal umbilical cord blood. Methods Umbilicus cord blood of neonates who were born to HBV DNA negative ( 〈 100IU/ml) mothers and peripheral blood of normal adults were collected. The frequencies of pDC and the expression levels of CD80/CD83/CD86/TLR-9 on pDC were detected by flow cytometry and analyzed using FlowJo software. The frequency of CD41 CD25+ CD127low Treg were detected by flow cytometry and analyzed using FlowJo software. The level of IFN-y, 1FN-α,IL-6,1L-IO, IL-17A and Flt-3L in whole blood of two groups were detected by Luminex technology and FLEXMAP 3D analyzer. Two groups of samples were compared with Mann-Whitney test and t test. Results With the frequency of CD4+ CD25+CD127low Treg cells there was no significant difference between neonates (7.34±1.54) and adults (8.14±1.15),(t = 1.40,P=0. 144). The frequency of Linl- HLA-DR ± CD123 ± pDC of neonate (0.07 ±0. 04) was significantly lower than the normal adult group(0. 14 ±0.04, P = 0. 000). The positive expression rate of pDC surface molecules CD80,CD83 and CD86were significantly lower than those of normal adult ( P = 0. 000). The positive rate of TLR-9 was no significant difference between neonates ( 21.30 ± 22.50 ) and adults ( 34.27 ± 11.31 ) , ( t = - 1.26, P = 0. 220 ). The level of IFN- and IL-17A in the neonatal group were significantly lower than that in adult group ( P = 0. 003, P = 0. 010) ,but there was no statistically significant difference between the two groups in the content of IFN-α, IL-IO, IL-6 and Fh-3L. Coneulsions In the newborn pDC reduce the frequency,immature phenotype and lack of stimulate capacity may be associated with chronic infection of the newborn after HBVinfection,while Treg cells may be independent of the mechanism.
