2型糖尿病患者前蛋白转化酶枯草溶菌素9水平及其与糖脂代谢相关性研究

摘　　要：目的研究2型糖尿病患者前蛋白转化酶枯草溶菌素9（PCSK9）水平及其与糖脂代谢的相关性。方法对503例南京医科大学第一附属医院内分泌科2型糖尿病患者进行病史采集、体格检查、生化检测，用ELISA的方法检测空腹血清PCSK9的水平。结果2型糖尿病人群血清PCSK9水平呈偏态分布，平均值为（83．501±42．604）ng／ml，中位数为78．237ng／ml；相关及多元逐步回归分析发现血清PCSK9水平与年龄、三酰甘油、总胆固醇、低密度脂蛋白胆固醇（LDL—C）、血浆致动脉粥样硬化指数（AIP）、收缩压、空腹血糖等呈显著正相关（P〈0．05）。且与年龄、三酰甘油、AIP、LDL—C、总胆固醇等动脉粥样硬化危险因素关系更为密切。结论2型糖尿病患者血清PCSK9的水平呈偏态分布；PCSK9水平与动脉粥样硬化危险因素密切相关。Objective This study was designed to measure the distribution of fasting serum concentrations of proprotein convertase subtilisin/kexin 9 （ PCSK9 ） in type 2 diabetic patients, and to analyze the glucose and lipid metabolic factors. Methods Fasting blood samples were obtained from 514 type 2 diabetic patients who enrolled in the Department of Endocrinology and Metabolism of Jiangsu Province Hospital between January 2011 to November 2011. After history-taking, physical and biochemical examinations, serum PCSK9 was measured by a sensitive and specific sandwich ELISA method. Results The distribution of PCSK9 value was skewed in this population with an average of （ 83. 501 ±42. 604 ） ng/ml（ 15. 851-250. 862 ng/ml）, and with the median of 78. 237 ng/ml. There was no significant difference between the genders in the levels of serum PCSK9. Serum PCSK9 levels was positively correlated with multiple common risk factors of atherosclerosis including age, systolic blood pressure, total cholesterol, low density lipoprotein-cholesterol （ LDL-C ） , triglyeerides, atherogenic index of plasma （ AIP）, and fasting blood glucose in this population （ P〈0.05 ）. The result of multiple stepwise regression analysis showed that serum PCSK9 levels was more closely related with age, total cholesterol, AlP, LDL-C, and triglycerides. Conclusion The distribution of PCSK9 value was skewed in type 2 diabetes. PCSK9 levels is closely associated with a variety of atherosclerosis related-index.
