摘 要:目的:通过分析克林霉素主要目标菌的耐药性变迁,评价克林霉素在围手术期预防用药中的作用。方法:参照CLSI标准测定我院葡萄球菌和链球菌临床分离株对克林霉素的耐药性,并通过文献检索评价国家监测网发布数据中葡萄球菌与链球菌对克林霉素和万古霉素的耐药性。结果:我院临床分离葡萄球菌和链球菌对克林霉素总体耐药率分别为64%和77%;Mohnarin监测网数据葡萄球菌和链球菌对克林霉素总体耐药率分别为57%和70%;CHINET监测网葡萄球菌和链球菌对克林霉素总体耐药率分别为49%和75%;所有葡萄球菌和链球菌菌株均对万古霉素敏感。结论:克林霉素目标菌耐药率较高,其作为围手术期预防用药的价值需进一步观察和讨论。Objective: To evaluate the preventative effect of clindamycin against perioperative surgical site infections via analyzing the changes of drug-resistance of the target bacteria. Methods: Susceptibility rate and resistance rate of Staphylococcus and Streptococcus strains which were isolated during 2010 – 2013 in our hospital against clindamycin were determined according to CLSI methods. All the references issued by Mohnarin and CHINET about the resistance of Staphylococcus and Streptococcus strains against clindamycin and vancomycin were retrieved through CNKI databank. Results: In our hospital, 64% and 77% of Staphylococcus and Streptococcus isolates were resistant to clindamycin, respectively. And these data were 57% and 70% in Mohnarin, and 49% and 75% in CHINET, respectively. No isolate was resistant to vancomycin. Conclusion: The resistance rate of clindamycin to the target bacteria is high, the value for perioperative prophylaxis should be further evaluated and discussed.
