参芎葡萄糖注射液致过敏性休克1例  

One case of anaphylactic shock induced by Shenxiong glucose injection

摘　　要：1例40岁男性患者,因下腹及会阴胀痛伴间歇性血精2周入院。患者行心电图检查提示心肌缺血,给予参芎葡萄糖注射液(100 m L,ivgtt)改善心肌缺血,输注约10 min后患者出现胸闷、气促、咽部异物感等症状,HR 112次·min^(-1),R 24次·min^(-1),BP 76/42 mm Hg,考虑为过敏性休克,立即停用参芎葡萄糖注射液,并给予5%葡萄糖注射液(500 m L,ivgtt)、地塞米松磷酸钠注射液(5 mg,iv)、盐酸肾上腺素注射液(0.5 mg,im)、持续吸氧(4 L·min^(-1))等对症治疗。患者症状逐渐缓解,约30 min后不适症状消失。One 40-year-old male patient who suffered from lower abdomen and perineal pain with intermittent hemospermia for two weeks was admitted to the hospital. Myocardial ischemia was indicated by electrocardiographic examination, and the patient received Shenxiong glucose injection（100 m L, ivgtt） for improving myocardial ischemia. Ten minutes after the infusion, the patient developed chest tightness, dyspnea, throat itching, the respiration was 24 times per minute, the heart rate was 112 times per minute and the blood pressure was as low as 76/42 mm Hg, which was considered as anaphylactic shock. The Shenxiong glucose injection was stopped immediately. The patient was treated with 5% glucose injection（500 m L, ivgtt）, dexamethasone sodium phosphate injection（5 mg, iv）, adrenaline hydrochloride injection（0.5 mg, im） and oxygen therapy（4 L·min-1）. After 30 minutes, all of the symptoms mentioned disappeared.
