摘 要:目的:探讨患儿发生导管相关性放射根瘤菌血流感染的危险因素。方法:收集2012-02-2014-02我院1 014例使用过各类医用导管的患儿资料,回顾性统计其中发生放射根瘤菌血流感染的患儿数量、性别、年龄、导管留置时间、导管留置部位、入住儿科重症监护病房时间、呼吸机使用次数、基础疾病、营养不良、使用免疫抑制剂、使用糖皮质激素、CD4、CD8、CD4/CD8,采用多因素Logistic回归分析患儿放射根瘤菌感染的危险因素。结果:1014例使用过各类医用导管的患儿中共发生了26例放射根瘤菌血流感染,感染率为2.56%,患儿年龄<2岁、导管留置时间>5d、入住儿科重症监护病房时间>5 d、使用呼吸机≥2次、基础疾病≥2种、使用免疫抑制剂、使用糖皮质激素、CD4和CD4/CD8降低均是放射根瘤菌血流感染的危险单因素。通过多因素Logistic回归分析发现患儿年龄<2岁、导管留置时间>5 d、使用呼吸机≥2次、使用免疫抑制剂、使用糖皮质激素、CD4和CD4/CD8降低是发生导管相关性放射根瘤菌血流感染的危险因素。结论:对免疫力低下的患儿导管留置时间>5d和使用呼吸机≥2次是导致放射根瘤菌血流感染的主要危险因素。Objective:To analyze the catheter-related blood infection due to Rhizobium radiation (R. radiobact- er) in pediatric patients. Method.. Clinical data of 1 014 pediatric patients with catheter-related treatment were col- lected from February 2012 to February 2014, including age, time and site of catheter, stay time of ICU, ventila- tor, underlying diseases, malnutrition, immunosuppressive agents, corticosteroids, CD4, CD8, CD4/CD8, and Logistic regression analysis of risk factors for R. radiobaeter infection. Result: There were 26 R. radiobacter in- fection children in I 014 pediatric patients with catheter-related treatment, infection rate was 2.560%, aged 〈2 y, catheter time〉5 d, stay ICU time 〉5 d, ventilator ≥2 times, underlying diseases ≥2 species, treatment of im- munosuppressive agents and corticosteroids, CD4 and CD4/CD8 reduction were dangerous single factor of R. ra- diobacter blood infection. Multivariate Logistic regression analysis showed that aged 〈2 y, catheter time〉5 d, ventilator ≥2 times, treatment of immunosuppressive agents and corticosteroids, CD4 and CD4/CD8 reduction were the risk factors of catheter-related R. radiobacter blood infection. Conclusion:Catheter time 〉5 d and venti- lator ≥2 times were the important risk factors of catheter-related R. radiobacter blood infection for immunocom- promised children.
