补肾活血方对肝肾阴虚型红斑狼疮肾炎患者疗效及免疫功能影响

摘　　要：[目的]探讨补肾活血方对肝肾阴虚型狼疮肾炎(LN)患者治疗效果及免疫功能影响。[方法]36例肝肾阴虚型LN患者按随机数字表法分为基础治疗组及联合治疗组,两组均应用甲基泼尼松与环磷酰胺(CTX)冲击治疗,联合治疗组同时应用补肾活血方治疗,4个月后观察两组患者系统性红斑狼疮(SLE)疾病活动指数(SLEDAI)、补体C3、C4、24 h尿蛋白定量、内生肌酐清除率(Ccr)、血尿素氮(BUN)、CD3^+、CD4^+、CD8^+水平。[结果]联合治疗组患者的SLEDAI、24 h尿蛋白定量、尿素氮、日糖皮质激素用量均明显下降,CD8+水平下调,与基础治疗组比较有统计学差异(P<0.05)。[结论]补肾活血方能够显著改善肝肾阴虚型狼疮肾炎(LN)患者临床症状,减少糖皮质激素维持应用量,并可在一定程度上纠正免疫功能紊乱。[Objective] To observe the clinical curative efects of Bushen Huoxue decoction on immunological condition in patients with lupus nephritis（LN）of Liver-kidney Yin deficiency. [Methods] Thirty-six LN patients were randomly divided into foundation treatment group（n=18） and combined treatment group（n=18）; steroids and cyclophosphamide（CTX） were administered to all patients,while Bushen Huoxue decoction were only administered to patients in the mixed treatment group.After 3-month treatment,SLE disease activity index score（SLEDAI Score）,C3, C4, 24 h urine protein quantity,endogenous creatinine clearance rate（Ccr）, blood urea nitrogen（BUN）, T lymphocytes subsets（CD3＋、CD4＋、CD8＋）, daily dosage of steroids and ill-effects were detected in both groups.[Results] Compared with foundation treatment group, combined treatment group could significantly decrease SLEDAI score,24 h urine protein,urea nitrogen, daily dosage of steroids and ill-effects and down-regulate the level of CD8＋T cell（P0.05）. [Conclusion] Bushen Huoxue decoction can significantly ameliorate LN of liver-kidney Yin deficiency syndrome and reduce the dosage of steroids by correcting immune dysfunction.
