摘 要:目的探究高频振荡通气辅助治疗新生儿呼吸衰竭的临床效果。方法选取海口市妇幼保健院2011年6月—2014年12月收治的新生儿呼吸衰竭患儿96例,按入院顺序分为观察组和对照组,每组48例。观察组患儿给予高频振荡呼吸机进行辅助治疗,对照组患儿给予常规婴儿型呼吸机进行辅助治疗。比较两组患儿临床效果及并发症发生情况,治疗前后动脉血氧分压(Pa O2)、动脉血二氧化碳分压(Pa CO2)、氧合指数(Pa O2/Fi O2)、Pa O2/Pa CO2。结果观察组患儿临床效果优于对照组(u=2.758,P=0.006)。两组患儿治疗前Pa O2、Pa CO2、Pa O2/Fi O2、Pa O2/Pa CO2比较,差异无统计学意义(P>0.05);观察组患儿治疗48 h后Pa O2、Pa O2/Pa CO2高于对照组,Pa CO2、Pa O2/Fi O2低于对照组(P<0.05)。观察组患儿气胸、慢性肺疾病、颅内出血发生率低于对照组(P<0.05)。结论高频振荡通气辅助治疗新生儿呼吸衰竭患儿的临床效果确切,能有效改善患儿氧合情况,且并发症较少。Objective To investigate the clinical effect of high- frequency oscillatory ventilation in the adjuvant treatment of neonatal respiratory failure. Methods A total of 96 children with neonatal respiratory failure were selected in the Maternal and Child Care Service Center of Haikou from June 2011 to December 2014,and they were divided into control group and observation group according to visiting sequence,each of 48 cases. Children of control group were given routine baby- type breathing machine for adjuvant treatment,while children of observation group were given high- frequency oscillatory ventilation.Clinical effect and incidence of complications, Pa O2,Pa CO2,Pa O2/ Fi O2 and Pa O2/ Pa CO2 before and after treatment were compared between the two groups. Results The clinical effect of observation group was statistically significantly better than that of control group( u = 2. 758,P = 0. 006). No statistically significant differences of Pa O2,Pa CO2,Pa O2/ Fi O2 or Pa O2/ Pa CO2 was found between the two groups before treatment( P〉 0. 05); after 48 hours of treatment, Pa O2 and Pa O2/ Pa CO2 of observation group were statistically significantly higher than those of control group,while Pa CO2 and Pa O2/ Fi O2 of observation group were statistically significantly lower than those of control group( P 〈0. 05). The incidence of aerothorax,chronic lung disease and intracranial hemorrhage of observation group was statistically significantly lower than that of control group,respectively( P 〈0. 05). Conclusion High- frequency oscillatory ventilation has certain clinical effect in the adjuvant treatment of neonatal respiratory failure,can effectively improve the oxygenation function,and the incidence of complications is low.
