摘 要:目的观察非负重游泳运动对膝骨关节炎大鼠膝关节软骨YKL-40表达的影响,探讨非负重游泳运动防治膝骨关节炎的机制,为临床采用非负重游泳运动防治膝骨关节炎提供理论依据。方法将40只雄性SD大鼠随机分为对照组、模型组、实验Ⅰ组、实验Ⅱ组,每组10只,对照组大鼠不予以任何干预,模型组与实验组大鼠采用木瓜蛋白酶注射法建立膝骨关节炎模型。对实验组进行为期6周的非负重游泳运动后,对所有大鼠膝关节软骨进行肉眼及光镜下观察形态改变并评分、免疫组化法观察YKL-40的表达情况。结果模型组、实验Ⅰ组、实验Ⅱ组大鼠膝关节软骨大体观评分分别为:4.10±0.74,2.90±0.74,2.10±0.57;改良Mankin评分分别为:6.17±0.16,4.69±0.18,2.95±0.15;YKL-40表达的AOD值分别为:0.33±0.02,0.29±0.02,0.26±0.03。三组大鼠膝关节软骨大体观评分、改良Mankin评分、YKL-40表达的AOD值均逐渐降低(P<0.05)。结论早期康复运动可以改变YKL-40在膝骨关节炎大鼠膝关节软骨细胞中的表达,能够延缓膝骨关节炎大鼠膝关节软骨的进一步退变。ObjectiveThrough the aquatic exercise to observe effects of knee osteoarthritis on expressional rats of YKL-40 in knee articular cartilage, discuss the mechanism of aquatic exercise in treating knee osteoarthritis, and offer the theoretical evidence that aquatic exercise can be used in in clinical practice.Methods40 male SD rats were divided into 4 groups at random: control group, model group, experimental group one and experimental group two,n=10. Rats in the control group were fed routinely. Rats in the model group and experimental group were established into rat knee osteoarthritis model by papain injecting. Rats in the experimental group took aquatic exercise for six weeks. We observed pathological changes of knee articular cartilagin rats with eyes and optical microscopes, then scored, and observed the expression of YKL-40 in the cartilage in rats by using immunohistochemical technology. ResultsThe model group, experimental group one and experimental group two’s scores of pathological changes were 4.10±0.74, 2.90±0.74, 2.10±0.57; the improved Mankin scores were 6.17±0.16, 4.69±0.18, 2.95±0.15; the AOD scores of the expression of YKL-40 were 0.33±0.02, 0.29±0.02, 0.26±0.03. The scores of pathological changes, the improved Mankin scores and the AOD scores of the expression of YKL-40 in the cartilage in these groups were decreased gradually (P<0.05).ConclusionEarly rehabilitation exercise can change the expression of YKL-40 in the cartilage in knee osteoarthritis rats, and delay the articular cartilage damage in knee osteoarthritis rats.
