胫后动脉穿支岛状皮瓣治疗跟腱外露创面

摘　　要：目的探讨胫后动脉内踝上穿支岛状皮瓣的设计和切取及其在跟腱外露创面修复中的应用价值。方法2009年8月-2013年8月，应用胫后动脉内踝上穿支岛状皮瓣修复跟腱周围皮肤缺损17例。结果本组17例皮瓣全部成活，术后随访6~12个月，受区修复后外形良好，组织质地柔软，无皮肤溃疡，足踝部功能恢复良好，行走正常。结论胫后动脉内踝上穿支皮瓣不损伤小腿主要血管，穿支血管位置恒定，血供可靠，成活率高，是修复跟腱外露创面的理想供区。Objective To discuss clinical application of pedicle perforators flaps of the posterior tibial artery to repair achilles tendon exposed wounds. Methods 17 pedicle perforators flaps of the posterior tibial artery were used to repair achilles tendon exposed wounds from August 2009 to August 2013, Results All 17 flaps survived completely. The follow- up period ranged from 6 to 12 months. Satisfactory clinical results were obtained-good shape after repair, tissue soft, good foot and ankle function. No ulceration was found at the recipient site. Conclusion The advantages of the perforators flaps of the posterior tibial artery include a constant vascular anatomy, highly vascularity and suitable for repair of achilles tendon exposed wounds.
