摘 要:目的分析北京市密云县腹泻患者沙门菌感染的病原学及耐药特征。方法以密云县医院和密云县中医院为哨点医院,采集腹泻患者的粪便标本,进行沙门菌分离培养,血清型鉴定,利用肉汤微量稀释法检测抗菌药物最小抑菌浓度(MIC),判定菌株的耐药情况。结果共监测腹泻患者605例,分离沙门菌35株,分离阳性率为5.8%;沙门菌流行呈规律的季节性于5月达到高峰;阳性患者主要分布在16岁~25岁和26岁~54岁年龄组。沙门菌对萘啶酸、四环素耐药率较高,对头孢西丁和头孢噻肟耐药率较低。34.3%的沙门菌对3种及3种以上抗菌药物耐药,鼠伤寒沙门菌耐药率比其他型别菌株高。结论沙门菌是密云县常见的主要致病菌,存在多重耐药菌株,应警惕耐药沙门菌形成的潜在暴发。加强以实验室为基础的主动监测对控制食源性感染性腹泻具有重要的意义。Objective To investigate the etiological characteristics and antibiotic resistance of Salmonella in diarrhea cases in Miyun,Beijing. Methods Stool specimen of diarrhea patients were collected from Miyun Hospital and Miyun Hospital of Traditional Chinese Medicine for the seperation and culture of Salmonella and serum typing. The minimal inhibitory concentration( MIC) of antibiotics was determined by micro- dilution method,so as to identify the drug resistance of bacteria. Results 35 Salmonella were isolated from 605 diarrhea specimens with positive rate of 5. 8%. Salmonella epidemic showed seasonal pattern with peak month in May. Positive cases mainly fell in age groups of 16- 25 years old and 26- 54 years old. The resistance rate of Salmonella to nalidixic acid and tetracycline were high,but the resistance rate to cefoxitin and cefotaxime were low. 34. 3%of Salmonella were multiple drug resistant to 3 kinds of antimicrobial agents or above. Drug resistance rate of Salmonella typhimurium was higher than other types. Conclusion Salmonella was the main pathogenic bacteria in Miyun among multiple- drug-resistant strains with peak moth in May and June. We should be alert to the early warnings on potential outbreaks of multiple-drug- resistance. Active surveillance system based on public health laboratory should play an important role in the control of food- borne infectious diseases.
