摘  要:目的从髓鞘修复的角度探讨电针治疗坐骨神经损伤(SNI)的机制。方法选SD大鼠随机分为5组:正常组、假手术组、模型组、模型对照组、电针组。除正常组和假手术组外,采用坐骨神经夹持损伤法建立SNI大鼠模型。电针组采用电针仪治疗,电流2 m A,频率为5 Hz,疏密波,选取的穴位为伤侧环跳、殷门、阳陵泉和承山。模型对照组造模后,与电针组治疗同时开始束缚大鼠。治疗每天1次,每次10 min,10次为1个疗程,共2个疗程。正常组、假手术组、模型组不干预。用髓鞘染色法观察脊髓及坐骨神经损伤点髓鞘的损伤及修复情况,用酶联免疫吸附法(ELISA)检测血清中髓鞘碱性蛋白(MBP)及其抗体的表达量。结果干预20次后,模型组、模型对照组白质中髓鞘松散,髓鞘化程度非常低;电针组髓鞘化程度较高,髓鞘、轴突结构完整。模型组及模型对照组血清中MBP、MBP抗体浓度明显高于正常组(P<0.05),电针组明显低于模型组及模型对照组(P<0.05),电针组MBP、MBP抗体表达量与正常组接靠近,稍高于正常组。结论电针可以降低MBP在血清中的表达,从而减少髓鞘、轴突崩解;电针可以降低MBP抗体在血清中的表达,减少大鼠自身免疫反应,从而减少对髓鞘的二次损伤,保护髓鞘,进而促进损伤的坐骨神经修复。Objective To explore the effects of electroacupuncture( EA) on sciatic nerve injury( SNI)from the perspective of myelin sheath repair. Methods Sprague-Dawley( SD) rats were randomly divided into 5 groups,including normal group,sham-operation group,model group,model control group and EA group. SNI model was established by clamping sciatic nerve in all groups except normal group and sham operation group. EA group received treatment using EA device with electrical current set at2 m A and 5HZ in dilatational wave mode. Points used were Huantiao( GB30),Yinmen( BL37),Yanglingquan( GB34) and Chengshan( BL57) on the affected side. Rats in the model control group were treated for 10 minutes once a day for two treatment courses with one course lasting ten sessions,starting from when electroacupuncture was offered in EA group. No intervention was offered to the normal group,sham operation group and model group. Injury and repair of spinal cord and sciatic nerve myelin sheath were observed after tissue staining. Enzyme-linked immunosorbent assay( ELISA) was used to detect the expression level of Myelin Basic Protein( MBP) and MBP antibody in the serum. Results After 20 sessions of intervention,myelin in the white matter of spinal cord appeared to be loose in the model group and model control group with lower degree of myelination while the structural integrity of myelin and axons was maintained in EA group with higher degree of myelination. The concentration of serum MBP and MBP antibody in the model group and model control group were significantly higher than those in the normal group( P〈0. 05). The concentration of MBP and MBP antibody in EA group were significantly lower than those in the model group and model control group( P〈0. 05). The expression of MBP and MBP antibody in EA group were close to yet slightly higher than those in the normal group.
