FTY720联用CsA在类风湿性关节炎中的治疗作用

摘　　要：目的探讨FTY720、环孢菌素A(Cs A)及两药联合应用对弗氏完全佐剂诱导性大鼠关节炎模型的治疗效果。方法 60只大鼠按照随机数字表法分为5组:正常对照组、模型对照组、FTY720治疗组、Cs A治疗组及FTY720+Cs A治疗组,每组12只。ELISA法定量检测关节液中TNF-α和IL-1β含量;28 d时处死大鼠取大鼠胸腺、脾脏,称重后评价胸腺指数及脾脏指数。结果 (1)HE染色结果:FTY720治疗组和Cs A治疗组与模型对照组相比,可见血管翳形成,但管腔略小,未见炎细胞形成;FTY720联合Cs A组被覆滑膜细胞下可见纤维组织增生,未见血管翳形成。(2)胸腺指数、脾脏指数及TNF-α、IL-1β含量:FTY720治疗组和Cs A治疗组较正常对照组升高,且差异有统计学意义(P<0.05),与模型对照组相比含量降低;FTY720联合Cs A组与正常对照组相比,差异无统计学意义(P>0.05),与模型对照组相比,差异均有统计学意义(P<0.05)。结论 FTY720联合Cs A可通过降低大鼠RA模型的胸腺指数、脾脏指数及TNF-α、IL-1β在类风湿性关节炎中起到治疗作用,但两者单独应用效果略差。Objective This work was to further understand the therapeutic effect of FTY720 in combination with Cs A on Freund ＇s adjuvant induced arthritis rat model. Methods 60 Wistar rats were divided into 5 groups of 12 and labeled as follows： Control,Model, FTY720 Treatment Only, Cs A Treatment Only, FTY720 ＋ Cs A Treatment. Ankle synovium samples were collected on the28 th day and tested for TNF-α and IL-1β levels through ELISA test. The rats were sacrificed after data collection, and under sterile environment, the thymus and spleen were collected, weighed and recorded for later calculation of thymus and spleen index.Results（1） HE Staining. Newly formed capillaries within a small cavity can be found in FTY720 Treatment Only and Cs A Treatment Only group when compared with the model group, FTY720 ＋ Cs A Treatment showed newly formed fibril tissues but no capillaries can be found.（2） Thymus and spleen indexes and blood serum TNF-α and IL-1β level test. Compared to the control group, both FTY720 treatment only group and Cs A treatment only group have higher index values, and the difference was statistically significant（P〈0.05）. However, when both groups were compared to the model group, the index values were lower and thus held no statistical significance（P〉0.05）. The index difference between FTY720 ＋ Cs A treatment group and the control group had no statistical significance（P〉0.05）, but showed statistical significance when compared to the model group（P〈0.05）. Conclusion Arthritis treatment using FTY720 only or Cs A only was not as effective as the treatment of arthritis using FTY720 ＋ Cs A. The FTY720 ＋ Cs A treatment of rheumatoid arthritis can suppress autoimmune response more effectively, and reduce the level of body inflammation factors and the swelling degree in the joints.
