软性膀胱镜检查对前列腺增生患者前列腺特异性抗原的影响

摘　　要：目的评估软性膀胱镜检查对前列腺增生(BPH)患者血清前列腺特异性抗原(PSA)的影响。方法选取自2010年6月至2014年6月山西医科大学第一医院泌尿外科进行软性膀胱镜检查的BPH患者56例,分别于检查前、检查后1 h及24 h检测血清PSA,均数比较使用配对t检验。结果软性膀胱镜检查后1 h前列腺增生患者的总PSA(t PSA)值增高(0.12±0.07)ng/ml、变化值/操作前t PSA为(1.66%±0.64%)、游离PSA(f PSA)值增高(0.08±0.04)ng/ml、变化值/操作前f PSA为(4.54%±1.95%)、f/t PSA变化值为(3.62%±1.21%),差异具有统计学意义(P<0.05);检查后24 h血清t PSA值增高(0.09±0.05)ng/ml、变化值/操作前t PSA为(1.58%±0.57%)、f PSA值增高(0.04±0.03)ng/ml、变化值/操作前f PSA为(2.73%±1.03%),差异具有统计学意义(P<0.05),f/t PSA变化值为(1.55%±0.35%)。结论软性膀胱镜检查会使BPH患者血清PSA值增高,但增高值占PSA比例很小,因此不会产生具有临床意义差异的影响。Objective To evaluate the impact of flexible cystoscopy on the total and free prostate-specific antigen（PSA） and free-to-total PSA ratio in benign prostatic hyperplasia（BPH） patients.Methods A total of 56 BPH patients from June 2010 to June 2014 were treated with flexible cystoscopy.The patients＇ blood were taken for PSA measurement before flexible cystoscopy and 1 hour and 24 hours after operation.The mean PSA values were compared using the paired t test.Results The t PSA,variation/t PSA,f PSA,variation/f PSA and percent-free PSA values increasing at one hour after flexible cystoscopy were（0.12 ±0.07） ng/ml,（1.66%±0.64%）,（0.08±0.04） ng/ml,（4.54 %±1.95%）,（3.62%±1.21%）,and they were significantly different（P〈0.05）.The t PSA,variation/t PSA,f PSA,variation/f PSA and percent-free PSA values increasing at 24 hours after flexible cystoscopy were（0.09 ±0.05） ng/ml,（1.58%±0.57%）,（0.04±0.03） ng/ml,（2.73%±1.03%）,（1.55%±0.35%）.The differences of（0.09±0.05） ng/ml,（1.58%±0.57%）,（0.04 ±0.03） ng/ml and（2.73 % ±1.03%） were statistically significant（P 〈0.05）.Conclusion Significant differences were noted in the PSA levels before and after flexible cystoscopy,but the percentages of these variations were quite small so they have no clinical value.
