视网膜光凝术联合中药治疗糖尿病性视网膜病变Ⅲ期60例临床观察

摘　　要：目的:观察自拟活血化瘀益气养阴方加减对非增生性糖尿病性视网膜病变(NPDR)激光光凝术后的视网膜病变的治疗效果。方法:采用美国莱特美532激光机行全视网膜激光光凝术加服自拟活血化瘀益气养阴方加减治疗60例108眼为综合组,与单纯光凝术治疗的60例107眼为激光组进行对照,两组均治疗3个月。结果:综合组视力明显提高,眼底视网膜出血、渗出、水肿吸收良好,眼底血管造影检查提示视网膜无灌注区消退,指比治疗前面积减少明显,治疗效果明显优于激光组P<0.05,差异具有显著性统计学意义。结论:自拟活血化瘀益气养阴方加减联合视网膜病变激光光凝术治疗NPDR,能明显改善眼底视网膜出血、渗出、水肿,提高视力。且自拟活血化瘀益气养阴方加减能改善光凝术有创性治疗引起的视网膜血流动力学改变和水肿。Objective： To observe the therapeutic effects of a self-formulated huoxiehuayu and yiqiyangyin recipe revised by addition and subtraction in the treatment of non-proliferative diabetic retinopathy（NPDR） after laser photocoagulation operation. Methods： 108 eyes （n=60） in the comprehensive treatment group received self-formulated huoxiehuayu and yiqiyangyin recipe revised by addition and subtraction in combination with pan-retinal laser photocoagulation applied via a laser machine 532 made by US lighted, 107 eyes （n=60） in the control group received only photocoagulation, both groups were treated for three months. Results： After the treatment, for the comprehensive treatment group patients, their vision improved significantly, their bleeding, effusion and edema absorption in eyeground retina relieved significantly, eyeground angiography indicated that non-perfusion areas from retina subsidized and decreased more significantly than situation before the treatment. Its efficacy was significantly better than that of the laser group （x2=6.496, P〈0.05） and the difference in efficacy between two groups was statistically significant. Conclusion： The self-formulated huoxiehuayu and yiqiyangyin recipe revised by addition and subtraction in combination with pan-retinal laser photocoagulation could significantly relieve bleeding, effusion and edema in eyeground retina and raise vision in treating NPDR. Moreover, it could improve hemodynamic changes and edema from retina induced by traumatic treatment during laser photocoagulation operation.
