摘 要:目的:探讨不同血糖水平对2型糖尿病(T2DM)整体认知功能、认知功能域、海马体积的影响。方法:对178例无合并脑血管病变的T2DM患者进行认知功能的测评、海马体积测定,比较不同血糖水平组间认知功能及海马体积差异。严格控制血糖,随访1年,复测认知功能及海马体积,比较治疗前后两者的改变情况。结果:1T2DM组与对照组的认知功能差异有统计学意义(P<0.01),其视空间、语言、注意力、延迟回忆等方面显著下降(P<0.01);2不同血糖控制水平认知功能域的差异有统计学意义(P<0.05),而血糖控制良好者语言、延迟回忆也显著减退(P<0.05);3T2DM组海马体积小于对照组,血糖控制不良亚组左侧海马体积显著缩小(P<0.01)。结论:T2DM具有独立于脑血管病的认知功能损害,尤其是血糖控制不良者;T2DM认知功能损害与海马体积改变相关。Objective:To explore the influence of different levels of blood glucose in type 2 diabetes mellitus (T2DM) on the cognitive function, cognitive domain function and hippocampal volume. Methods:Monitored 178 patients with T2DM without cerebrovascular diseases, and compared cognitive differences and hipp-ocampal volume in different levels of blood glucose groups. Patients with strict control of blood glucose were followed up for one year. Cognitive function and hippocampal volume were compared pre- and post-treatment. Results. ①The visual-spatial, language, attention and delayed recall were significantly declined in T2DM than in the control( P〈0.01). ② Differences between cognitive domain function of different levels of blood glucose were statistical significant( P〈0.05). ③T2DM hippo-campal volume was less than the normal control group, the left hippocampal volume in the blood sugar uncontrolled group reduced significantly. Conclusion:Cognitive dysfunction existed in T2DM before cerebrovascular damages, especially in patients with uncontrolled blood sugar values. T2DM cognitive impairment associated with changes of hip-pocampal volume.
