改良单孔腹腔镜阑尾切除术疗效观察

摘　　要：目的评价改良单孔腹腔镜阑尾切除术（modified single-port laparoscopic appendecto—my，MsLA）的临床疗效及安全性。方法对2011年11月至2014年10月同期进行的75例MSLA病人与65例常规腹腔镜阑尾切除术（1aparoscopic appendectomy，LA）病人的临床资料进行比较，分析MSLA病人的临床疗效及安全性。结果MSLA组、LA组病人平均手术时间分别为（41．32±2．51）min和（39．21±7．31）min，术中出血量分别是（33．51±3．35）ml和（35．42±4．59）ml，肛门排气时间分别是（47．32±9．52）h和（46．21±6．02）h，住院费用分别为（8038．34±105．21）元和（8051．52±93．15）元，各指标差异均无统计学意义（P〉0．05）。MSLA组、LA组在术后疼痛缓解时间分别为（7．32±0．52）h和（51．52±2．15）h、住院时间分别是（38．34±5．21）h和（51．52±2．15）h，差异均有统计学意义（P〈0．05）；MSLA组病人美容效果明显优于LA组。安全性方面：两组均无出血、切口感染、切口疝、粘连性肠梗阻、残株炎、粪漏等并发症。结论改良单孔腹腔镜阑尾切除术具有疗效确切、安全、美容、痛苦轻、康复快等优点，值得推广应用。Objective To evaluate the therapeutic efficacy and security of modified single-port laparoscopic appendectomy （MSLA）. Methods Bewteen 2011. 11 and 2014. 10, a total of 140 patients with acute appendicitis admitted to our department for appendectomy were categorized into a modified single-port laparoscopic appendectomy group and laparoscopic appendectomy group. The clinical outcomes and hospital costs were assessed and compared. Results The two groups were similar in terms of demographics, and comorbidities. There was no significant difference in operation time, bleeding volume, postoperative ambulation time, anal exhaust time and hospitalization expenses between two groups （P〉0. 05）. The MSLA group has a less postoperative pain （7. 32 ± 0. 52 vs. 19. 21 ±1.02, P〈0. 05） and shorter length of hospital stay （38. 34 ±5.21 h vs. 51.52 ± 2. 15 h） than LA group. The cosmetic effect in MSLA group was better than that of LA group. No bleeding, wound infection, incisional hernia, adhesive intestinal obstruction, stump inflammation, fecal leakage and other complications occurred in both two groups. Conclusions With the advantages of definite curative effect, safety, beauty, small trauma, less pain, faster recovery, and shorter hospitalization time, MS- LA is worthy for popularization and application.
