益气活血颗粒防治腹膜透析患者心血管并发症的临床观察

摘　　要：目的:观察益气活血颗粒对腹膜透析(PD)患者心血管并发症的影响。方法:共37例患者,治疗组18例,对照组19例。在常规PD和西医对症治疗的基础上,治疗组加用益气活血颗粒(药物组成:生黄芪颗粒20 g、三七颗粒3 g、丹参颗粒10 g、薤白颗粒10 g),疗程6个月。观察心血管疾病预测指标、心脏彩超指标及相关实验室指标的变化,以及心血管并发症的发生情况。结果:(1)治疗组治疗后血清Hcy、Cys C、TNF-α下降(P<0.01),LVEF、E/A、Alb上升(P<0.01,<0.05,>0.05),对照组治疗后男性LVMI上升(P<0.05),组间上述指标变化差异有统计学意义(P<0.05,<0.01)。(2)治疗组治疗后LVH和CHF发生率减少,CVC增加,对照组LVH、CHF增加,但组间差异无统计学意义(P>0.05)。结论:益气活血颗粒有降低PD患者血清Cys C、Hcy水平的作用,并有助于改善左心室收缩、舒张功能,减少LVH和CHF的发生,其作用与血清TNF-α、Alb水平有关。Objective： To observe the effect of Yiqihuoxue Granule on prevention and treatment of cardiovascular complications in peritoneal dialysis（ PD） patients. Methods： 37 patients were divided into treatment group（ 18 cases） and control group（ 19 cases）. Control group received conventional PD and western medicine,while the treatment group was added Yiqihuoxue Granule（ which is composed of Shenghuangqi granule 20 g,Sanqi granule 3 g,Danshen granule 10 g,Xiebai granule 10 g） as well for six months.After treatment,changes in cardiovascular disease predictors,echocardiography indicators,other related laboratory indicators,and the occurrence of cardiovascular complications were observed. Results：（ 1） After treatment,Serum Hcy,Cys C,TNF- α decreased（ P 0. 01）,LVEF,E / A,Alb increased（ P 0. 01, 0. 05, 0. 05） in treatment group while LVMI（ male） increased（ P 0. 05） in control group. There was statistical significance between two groups before and after treatment（ P 0. 05, 0. 01）.（ 2） Compared with that before treatment,the incidence of LVH,CHF decreased,CVC increased in treatment group,while LVH,CHF increased in control group. The difference of two groups has no statistical significance（ P 0. 05）. Conclusion： Yiqihuoxue Granule can lower PD patients＇ serum Cys C and Hcy levels,improve systolic and diastolic function of left ventricular and reduce the occurrence of LVH and CHF,which plays an important role in regulating serum TNF- α and Alb level.
